Employee was attempting to clear drill steel of clogged material when compressed air was released from the charged drill steel. This resulted in a cut to the bottom of the miner's thumb/palm and pelting of the top of the miner's right hand with dust/sand/stone chips.

Employee was sitting on the screen deck checking on the crew. When EE stood up to go get some bolts for them, EE felt stiffness in the left knee.
Employee was enlarging an existing hole in a section of rubber (conveyor belting) with a utility knife. In the process of making the hole the knife slipped and cut their hand resulting in 6 stitches.

The employee picked up a roller when EE lost EE's grip. It slipped out of EE's hands, and the shaft's keyway cut the ring finger on EE's right hand. The worker received four stitches to close the wound.

Employee tripped getting into cab and fell with hands forward into cab. Employees right hand middle finger contacted cab floor first and dislocated to the right.

Employee was working to remove the well at the re-crush plant. The well got hung up on a collar. Employee used a hammer and wedge to remove collar. Once collar got knocked free the well casing slid down and pinched employee's left hand ring finger tip.
Employee was walking to car, slipped and fell. Tweaked ankle/foot/knee & hurt right hand/elbow. *This is not a reportable injury. The employee has only received first aid. I am reporting to avoid citation if the employee does receive more than first aid outside of the 10 day reporting requirement (as advised by an inspector over the phone).

Employee was exiting M3 Chute after cleaning an overflow. Pant leg got caught on the chute lip and caused employee to lose balance. In regaining balance, employee tweaked back.
Employee was shoveling material into wheelbarrow. When employee went to empty wheelbarrow, felt a tweak in right shoulder. Full rotator cuff tear.

Employee was changing a M1 gear. It was raining and employee's safety glasses fogged up multiple times. Employee hung glasses on shirt and tapped the gear in place and felt something in eye.
Employee was checking the main conveyor over the portable screen and as EE was climbing down, EE hyper-extended right shoulder backwards and felt a sharp pain. First aid was administered. Employee went to doctor on 10/13/16 and received two prescriptions.
Employee was asked to cut down a pine tree near the primary. Employee cut a wedge on the back side of the tree. As employee was cutting through the tree, the chainsaw kicked back and lacerated left knee.

While an employee was lifting safety grating his foot slipped striking his shin on one of the divider plates within the chute.
While swinging a sledge hammer to clean process chute, employee's finger got caught between the sledgehammer handle and the angle-iron of the chute. Employee reported the incident to his supervisor at lunchtime. Employee went to the hospital on 6/5/15, where it was determined that the employee has dislocated and fractured his finger.
Employee stepped down off loader ladder and felt pain in lower back. Employee examined at medical facility on Monday, May 13, administered a prescription and released to light duty work.

While working in a transformer vault and installing a 3-inch high voltage electrical cable, he started to adjust the cable supports. The cable slid back into the 5" conduit pulling his hand into it. Finger was sore and ice applied. An hour later, he went to squeeze a hook and felt a pop in his finger.

While observing cutting and grinding being done in screen house, it was very windy and particles of debris flew into his right eye. Was wearing safety glasses. Washed eye thoroughly. But 3 days later felt pain in eye and blurred vision. Sought medical care. Infection from small rusted particle.

Employee was descending the ladder of his front end loader when his left boot heel got caught on the step "tweaking" his knee. This led to pain/tightening of his inside back of the knee and tenderness to the kneecap.
Employee had just decended a fixed ladder from the rock grab next to the primary. When he reached the floor level, he took one step away from the ladder, when his left foot / leg went through the floor. It gave way beneath him, resulting in him striking against the floor, causing pain in his left hip and left shoulder.

Claimant was on a vehicle when he twisted and hurt his back.
Employee states he twisted right knee sometime during shift, nothing to pinpoint the accident, just that he had pain and it worsened towards end of shift.
The employee had stopped the fuel truck that he was driving on a flat, clear, dry area of the quarry floor. He dismounted the cab of the truck and when his right foot hit the ground, as he exited the vehicle, he experienced pain in his lower back.
Employee was attempting to drive shaft out of sprocket/shaft assembly. To do this it was required that he lift this assembly onto two railroad ties to allow room for shaft to be driven out of sproket when hit from above. When employee set sprocket onto tie #2 he mis-judged the location of the tie and caught thumb between sprocket and railroad tie.
Was assisting another employee who was welding and while holding steel, glove caught fire causing burn to pinky finger.

WHILE SHOVELING, MUSCLE IN CHEST WAS STRAINED. EE GIUVEN IBUPROFEN & PLACED ON LIMITED DUTY FOR ONE WEEK.
WHILE DESCENDING LADDER OF LOCOMOTIVE WITH ARMS SLIGHTLY ABOVE HEAD LEVEL, EE TURNED HIS HEAD SLIGHTLY AT THE SAME TIME & FELT A POP INHIS NECK. EE DIAGNOSED WITH A CERVICAL STRAIN & PLACED ON LIMITED DUTY.

EMPLOYEE WAS TIGHTENING WEDGES ON THE 4 1/4" CRUSHER AND LOST HIS FOOTING STRIKING HIS BACK ON THE CATWALK. THE EMPLOYEE WAS THEN STRUCK BY HIS FALLING SLEDGE HAMMER ON HIS LEFT HIP.
WHILE GRINDING THE BIN WALL, DIRT & METAL PARTICLE LODGED IN EE'S LEFT EYE. EYE PROTECTION WAS BEING WORN, BUT THE PARTICLE ENTERED FROM THE SIDES. EE FELT DISCOMFORT THE NEXT DAY & WENT TO HI S PERSONAL PHYSICIAN.
