Miner was on first floor of "New Office" working on a ladder changing a light. Miners co-worker went to basement to retrieve another new light. Co-worker heard a thump and a bang on the ceiling. Co-worker found miner sitting slumped on the floor unresponsive. Started CPR, called 911, deployed AED but the miner could never be revived.
The employee was performing routine preventative maintenance on a transformer when EE was fatally electrocuted.
Arc flash event involving electrical equipment that resulted in burns to the employee's arms and torso as well as electrical equipment damage. The employee was injured and hospitalized after the incident but, passed away in the hospital on 08/22/2024.

An employee operating a Caterpillar 770 haul truck hauling material to repair the road to the stripping area. As the employee dumped the load the bed contacted overhead powerlines. The employee exited the truck to extinguish a fire. Upon returning to the truck EE reached for the handrail. When EE touched the truck, EE was electrocuted.
Investigation is still ongoing, and an autopsy is being performed to determine the cause. *Per the prelim: Two contractors were electrocuted when a 14,400-volt overhead power line came in contact with their off-road dump trucks, and the contractors exited their trucks and contacted the ground.

An examiner was found unresponsive in an underground sump. As of this time the exact cause of the employee's death is still undetermined.
On 09/01/2022 at approximately 4:40 p.m. at 32 brk on 2 Belt track, employee came into contact with a 480 volt roof bolter cable and was fatally injured.
Contributing Factors: (1)Energized equipment. (2)EE failure to follow company LO/TO procedures. (3)Outer wire jacket showed signs of heating & cooling. (4)EE grabbed the live "C" phase wire.
Employee was working in a de-energized 4160 volt GFCI controller in an attempt to reverse the polarity of the wires when it somehow became energized.
EE was attempting to identify the problems with the scrubber motor on the JM 6911 continuous miner on the number one section in the number one entry when EE came in contact with electricity. The employee was brought to the surface and transported by ambulance to a Hospital where EE later passed away from the injury sustained from the incident.
A contractor died at the operation underground. The cause of death is unknown at the time of this report. MSHA issued a 103(k) order on the area. The information in this report is all we have at this time.
EE was working to replace a high voltage starter for the slope belt and was in the process of adjusting the mechanical linkage connected to the switch when EE came in contact with electricity. EE was transported via ambulance to CAMC Memorial where EE passed away from the injuries sustained from the incident.
Employee was electrocuted by transformer power.
Temporary contracted employee contacted the 12500 volt overhead power line with an irrigation pipe
Employee went to prime the diaphragm pump located outside the stripping machine building, adjacent to the hydraulic room. Eight minutes pass where EE is unaccounted for prior to being found unresponsive, kneeling in liquid, face against the stainless steel flange of the tank house return HDPE line.
The employee was working on a Fletcher roof bolter. He had the panel board off trouble shooting inside the electrical components of the machine. He apparently came in contact with electricity. The investigation is not complete at this time.
Employee pulled aluminum boat over three electric conductors in dredge pond. A patch on the bottom of the boat cut into one conductor and electrocuted the employee. The employee died as a result of the accident.
Employee was working near an energized Soft Start Component. Started convulsing or having a seizure. Possible electrocution. Still unknown.
Electrician was performing work on a shuttle car cable reel on the West Main Left section when his received a fatal electrical shock. The electrician came in contact with the exposed 995 volt energized cable leads in the cable reel compartment.
Co-worker was in the process of shutting down the generators, to do maintenance to the power cables. At that time employee started to unguard the power cables on the switch van out of the vision of co-worker. He refused to have the power checked by two other co-workers. Moments later he was electrocuted.
Employee was welding two pipes together at an elevated location when an apparent electrocution happened. He was working in a confined location and was probably wet with sweat.
The investigation into the accident is on-going, but as of now, it appears as the three employees were installing ground vault indicator lights when an ach flash occurred, injuring them. The employees were transported to the hospital.
Employee was connecting power cord that feeds the dredge.
Employee (electrician apprentice) and a co-worker (journeyman electrician) were working on a light fixture of the SX upper tailings booster when he came in contact with energized wire.
EE was found unconscious, with no resp or pulse pit between hydroseparator and floc building. CPR was administered and EE was transported to CMS where he was pronounced dead. Autopsy report has not been recieved. A heat trace tape was found arcing near where EE was found.
While atempting to repair a shuttle car without determining that the trailing cable was de-energized and without locking-out and tagging the power source. EE received fatal injries when he cut into an energized phase lead.
Boom truck being used to set concrete safety barriers beside driveway at the request of MSHA inspector. Boom assy. on truck had approx. 18' clearance under overhead power line. Operator of truck and deceased had just moved truck and reset up to have more clearance under the wires. On first lift the boom was extended too far and made contact with wires. Day was clear, dry and sunny.
An electrician that was trouble shooting an installation was electrocuted by a single leg of a 3 phase 4160 volt service.
The electrician was in the process of trouble shooting an electrical problem in the line of power feeding the 8200 Dragline. At the time of the accident the victim was in the KM-12 junction box when he contacted live electrical components resulting in fatal injuries.
Electrical explosion.
Employee of Adams Trucking was weedwacking brush. Blade on weedwacker hit guide wire for electrical pole. The wire was severed and the tension pulled it back toward the transformers on the pole. the guide wire came in contact with a bare wire. At this time we assume electricity went to ground causing fatality. Under Investigation.
Contractor (electrician) was working in building. It is unknown just what happened. The electrician contacted energized conductors while installing an electrical circuit.
A master electrician was fatally injured while trouble-shooting 460 volt electric motor. (question #26: experience at this mine TWO hours.)
The employee came into contact with a guy wire, which broke loose and came in contact with an energized condutor on the pole. There was no insulator or proper anchor on the guy wire, this caused the employee to be electrocuted.
The employee suffered a fatal electrocution injury while engaged in positioning the electrical MCC building. The boom truck became energized when contacting overhead power lines.
The victim was demonstrating the features of the fault wizard. During the demonstration in the mine office he received an electrical shock and collapsed.

EE was making repairs/troubleshooting inside an electrical box when he contacted an energized component.
VICTIM AND TWO EMPLOYEES WHERE IN THE PROCESS OF CHANGING OUT SURFACE HIGH VOLTAGE TRANSFORMERS. DURING THE PROCESS THE VICTIM CAME INTO CONTACT WITH THE HIGH VOLTAGE CURRENT, RESULTING IN INJURIES INVOLVING BURNS, CUTS AND BRUISES. VICTIM LIVED UNTIL 10/29/04 AT 3:00 P.M.
NO TAG LINE USED ON THE CONVEYOR BEING HOISTED. WORKING WITHIN 10 FEET OF ENERGIZED OVER HEAD LINE.
HE WAS ASSISTING A CERTIFIED ELECTRICIAN IN INSTALLING A 7200 VOLT PLUG (CATHEAD) WHEN HE APPARENTLY CAME IN CONTACT WITH AN ENERGIZED RECEPTACLE ON THE POWER DISTRIBUTION BOX (VCB). THIS RESULTED IN A FATAL.
EE was making electrical repairs to the load side loop on a deenergized pole mounted solid blade disconnect switch. He had finished the repair work and was starting to come down the pole. Some how he received an electrical shock and fell down the pole.
ELECTRICIAN WAS FATALLY ELECTROCUTED WHILE REPAIRING A MINER CABLE. THE CABLE WAS NOT LOCKED AND TAGGED OUT BY THE ELECTRICIAN.
BOOM TRUCK OPERATOR APPARENTLY SWUNG LOAD NEAR POWERLINE. EMPLOYEE LENDING A HAND TO UNHOOK LOAD WAS ELECTROCUTED.
EMPLOYEE WAS IN THE PROCESS OF REPAIRING #3 SHUTTLE CAR CABLE. POWER TO CABLE HAD BEEN TAGGED & LOCKED OUT. HE WAS DISCOVERED LYING ON HIS BACK & NOT BREATHING. CPR WAS ADMINISTERED AND HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS PRONOUNCED DEAD AT APPROX. 8:20 P.M. MSHA/STATE INVESTIGATIONS ARE INCONCLUSIVE AT THIS TIME.
WHILE TROUBLE SHOOTING MAIN OUTSIDE SUBSTATION, EMPLOYEE CAME IN CONTACT WITH HIGH VOLTAGE RESULTING IN DEATH.
EMPLOYEE (FOREMAN) DID NOT TURN POWER OFF AND LOCK OUT 486 VOLT ELECTRIC BOX. CUT CABLE AND MADEA SPLICE. WAS ELECTRICUTED. TAKEN TO MOUNT CARMEL HOSPITAL. WE WERE NOTIFIED OF HIS DEATH APPROX 2:00 PM.
BREAKER BLEW UP AND BURNED EMPLOYEE.
