MINER WAS USING A PRY BAR TO LIFT UP THE LATCH OF A PINTLE HITCH. THE PRY BAR LOST PURCHASE ON THE LATCH WHILE THE MINER WAS PUSHING DOWN ON THE END OF THE BAR RESULTING IN THE LOSS OF BALANCE. MINER FELL FORWARD AND STRUCK THEIR FACE ON THE TOP SUPPORT BAR OF A MUDFLAP RESULTING IN THE DISLODGEMENT OF A DENTAL CROWN. THE UNDERLYING TOOTH WAS CHIPPED AND STILL AFFIXED TO THE CROWN.

Employee cutting belting for back flaps for feed ends of screen. Employee had the belting on the ground cutting, while on knees the utility knife slipped from the rubber, causing small laceration to inner left side of right knee.
WHILE PERFORMING MAINTENANCE ACTIVITIES ON AN HP 400 CONE CRUSHER, TEAM MEMBER WAS REMOVING PIN FROM HYDRAULIC CYLINDER ON AN HP 400. WHEN PIN WAS REMOVED, THE HYDRAULIC CYLINDER CAME DOWN PINCHING LEFT PINKY FINGER BETWEEN CYLINDER AND OBJECT USED TO REMOVE PIN. FINGER REQUIRED STITCHES.
Employee was using a cheater pipe on a wrench to loosen a nut. When the bolt came loose the employee smashed hand between the pipe and excavator bucket. The employee received 2 stitches from the incident.
Employee crawled into a triple deck incline screen to change the poly deck screens. Employee was using a 2lb hammer to adjust the screen when the hammer bounced back hitting EE above EE's right eye.
Employee was cutting rubber belting material with a razor blade knife. Employee was pulling knife towards self when it slipped out of the rubber material and EE hit EE's upper left leg with the blade. Employee received a laceration approximately 1 inch deep and 1 1/2 inches long. Employee received sutures to close the wound. Minimal impairment.
EE was hitting a tooth on the loader bucket with a maul and a piece of metal came off going into EE's upper leg.

Employee was lowering light plant, crank handle was loose, got wrench to tighten bolt on handle, wrench slipped off bolt head, employee felt pop in right shoulder and pain.

Miner was using a sledgehammer and a punch to drift a pin out so that the tooth on an excavator bucket could be replaced. The miner hit their thumb with the sledge hammer when it glanced off of the top of the punch. The miner's right thumb suffered a fracture on the tip of it.
Employee was chipping hardened soil from the flange of a stacker inlet so that the flange could be changed. Employee was using a spud bar in a standing position above the inlet. On downward stroke spud bar hit rubber belting and bounced back up causing a small laceration above employees right eye. Employee was taken to medical clinic and received 7 sutures to close the laceration.
Employee was removing a bearing race from screen housing with a hammer tapping the race causing a piece of the race to chip off striking employee in the left leg above knee. The foreign body embedded under the skin.
Miner reported to employer on 09/29/2016 that activities on 05/24/2016 related to cleaning out a drive-over hopper with a long spade bar led to shoulder pain.

He was holding the knife in his right hand when the knife slipped out of the rubber material and back at him. The miner stabbed the palm of his left hand with the knife. Damage was limited to a small laceration.

Injury was not reported until 9/6/13. EE alleges that he felt a pull in his stomach while adjusting the under belt on the twin screen. He has been diagnosed with a hernia and released back to full duty but will require surgery at a later date. The accident date is a just a guess by the EE.

Employee was trying to clear a rock that was stuck in the jaw. He was using a wedge attached to a chain, standing on the platform above the jaw. The wedge bounced up and as it fell he was holding onto the chain and the weight pulled his shoulder.
Prying and pounding on a frost chunk on a field conveyor belt, the pry bar bounced off the belt and jarrred his wrist.
A large slab of rock had lodged itself inside the hopper at Separator #2. Plant was shut down and employee was using a pry bar to dislodge it when the rock came free. Other rocks followed causing him to lose control of the pry bar. The bar smashed his right hand into the side of the hopper, injuring his right middle finger.
Dozer operator was prying a rock out of the dozer arm and track roller. When the rock came out it splintered pieces off into his right eye.

Superintendent was wrenching on an adjusting nut on the conveyor tail pulley. The wrench slipped causing his right hand to jam into the metal guard and lacerating the end of the middle finger. Xrays revealed a fracture in the end of the finger as well.
EE was driving a wedge to split the top shell from the bottom shell when the wedge fractured and a piece of the wedge struck him just in front of the rt ear (temple area).
CRUSHER CHUTE PLUGGED UP. EE USED A 5 FT BAR TO TUG A PIECE OF CONCRETE FROM BEHIND. IT WAS PINCHED BETWEEN THE CHUTE AND GOOSENECK BOLT. THE PIECE CAME LOOSE AND THE CHUTE CAME DOWN ON THE BAR AND SPUN IT AROUND & HIT EE ON THE SIDE OF THE HEAD.
EE WAS UNPLUGGING THE JAW. A ROCK FELL AND SPLIT. EE GOT CHIP IN HIS EYE.
