Employee reached under a load suspended by an all terain fork lift when the load shifted and fell pinching the employee's left hand. The resulting injury to the palm side base of the employee's left thumb required three sutures to close.

Employee was operating a 966c loader. He went into the muck pile to get a bucket of material and the front wheels of the loader fell into a soft spot and came to a abrupt stop. The employee sustained a whiplash. He was wearing a seat belt. His conditioned worsened and he was given restrictions on 11/4/05.
EE claims that the brakes failed at the top of the slope leading into the pit. EE was on his way to get his 9th load of the night. EE had conducted pre-shift on the haul unit. The vehicle str4uck the highwall as he attempted to negotiate a curve and ramp leading out of the pit.EE received a laceration to left forehead area and was hospitalized.
Employee was being lowered in manbasket. Basket stopped suddenly. Employee was standing with legs crossed lighting a cigarette when basket stopped causing him to fall in squatted position. Knee struck basket interior causing injury.

EMPLOYEE WAS ATTEMPTING TO DRIVE VEHICLE AWAY FROM EMPLOYEE ROOM & ANOTHER VEHICLE WAS PULLING AWAY AT THE SAME TIME. 2ND VEHILCE SPUN WHEELS ON ICE AND DIRT FLEW IN THRU EE'S OPEN WINDOW & STRUCK HIM IN THE RT. EYE, CAUSING AN ABRASION.
EE CLAIMS BACK INJURY WHILE OPERATING AN R-35 HAUL TRUCK IN THE PIT. WHILE DRIVING INTO THE PIT, THE TRUCK STRUCK A POT HOLE, CAUSING THE SEAT TO RISE AND FALL SUDDENLY, RESULTING IN THE INJURY.
Spreading material on ramp to pit area. Motor on loader quit running due to low RPM & use of hydraulic controls. Operator lost use of the brakes & steering, loader traveled approx. 125' into the wall. The operators head struck the windshield & he was wearing his seatbelt during the accident.
Employee was driving off road truck. She said while being loaded a rock fell in the back of the truck and bounced her up. When she came down she hurt her back and is having numbness from hip to knee on the left side. She went to a doctor on the 17th. She is still out of work.

EE WAS LOADING ROCK IN THE PIT FROM THE MUCK PILE. HE GOT THE BUCKET FULL AND STARTED TO BACK UP WHEN A ROCK ROLLED DOWN THE PILE AND HIT THE LOADER BUCKET. THE IMPACT JOLTED THE LOADER CAUSING EE TO HAVE BACK PAIN.
Was being loaded and a rock fell out of bucket and jolted the truck. He felt pain in neck and his right arm and leg went numb.
Employee broke his finger when it became pinched between the walk behind bobcat loader and the handrail in tunnel.

EE WAS DRIVING MANLIFT TO JOB SITE WHEN HE HIT A HOLE THAT CAUSED HIM TO WHIP TO ONE SIDE.
TAKING MANLIFT OUT OF ROADWAY TO ALLOW LOADER TO PASS, LIFT HIT A BUMP CAUSING BASKET TO JUMP. THE SUDDEN MOVEMENT UP AND TO THE SIDE THREW THE EMPLOYEE TO SIDE OF BASKET. HE TWISTED RIGHT ANK LE AND STRAINED RIGHT SHOULDER. EMPLOYEE STAYED AT WORK - WENT TO DOCTOR ON 10-14-03 (AFTERNOON) & RETURNED TO WORK REGULAR SHIFT.
EMPLOYEE WAS OPERATING A WATER TRUCK AND WHILE DRIVING THE TRUCK OVER A PERIOD OF TIMME HIS LOWER BACK AND LEG BEGAN TO HURT. THIS DID NOT BECOME REPORTABLE UNTIL AUGUST 4, 2003.
EE WAS OPERATING THE CAT YARD LOADER. WHILE BACKING, THE LOADER ROLLED INTO A DEPRESSION, AND THE OPERATOR SAID IT "JARRED" HIS BACK, CAUSING PAIN. THE OPERATOR SAID HE HAD INJURED HIS BACK AT ANOTHER JOB AND THIS AGGRIVATED IT.
EE WAS OPERATING THE WATER TRUCK IN THE PIT. EE CLAIMED THAT THE WATER VALVE CONTROL BOX BROKE LOOSE & FELL ON THE FLOOR. AS HE ATTEMPTED TO MOVE THE BOX, HE TOOK HIS EYES OFF THE ROAD. THE TRUCK VEERED SHARPLY TO THE LEFT, AND STRUCK THE BERM. THE TRUCK PROCEEDED THROUGH THE BERM AND OVER THE WALL, APPROX. 20', COMING TO REST UPSIDE DOWN.
EE WAS TRAMMING THE R-50 HAUL TRUCK DOWN THE S. RAMP LOADED WHEN HIS BRAKES FAILED. HE DROVE TO THE NEXT BENCH THEN INTO THE BERM. THE TRUCK HITIT AT A PERPENDICULAR ANGLE CAUSING THE TRUCK TO JUMP THE BERM & STRIKE ANOTHER WHICH STOPPED THE TRUCK. THE OPERATOR WAS WEARING HIS SEAT BELT THUS KEEPING HIM IN THE TRUCK. X-RAYS SHOWED TWO FRACTURED RIBS. HE WAS RELEASED FOR LIGHT DUTY.
EMPLOYEE DROVE HAUL TRUCK OVER A BENCH WHICH RESULTED IN A LACERATION TO HIS LEG.
"WHIPLASH" NECK INJURY. EE WAS DRIVING WATER TRUCK, HIT BUMP, APPLIED BRAKES HARD, HAD MARKS TO WHERE TRUCK CAME TO A STOP. HE SAYS HE HEARD A "POP" IN HIS NECK AND FELT NECK PAIN. TRANSPORTED BY LOCAL RESCUE SQUAD.
EMPLOYEE'S BACK WAS INJURED AS A RESULT OF OPERATING A LOADER. REPORTED LATE DUE TO EMPLOYEE'S QUESTION AS TO WHETHER OR NOT INJURY WAS WORK-RELATED.
EE WAS GREASING NEW PLANT WHEN START UP ALARMS WENT OFF. EE WHO WAS NOT LOCKED OUT, WAS STANDING ON A CONVEYOR WHEN IT STARTED. THE EE WAS KNOCKED DOWN AND CARRIED INTO A CHUTE WHERE HE EXITE D AN 8 1/2"X 30"OPENING ONTO ANOTHER BELT. HE WAS SEEN BY CO-WORKERS AND THE BELT WAS STOPPED.EE SUFFERED BRUISES AND A BROKEN LEFT ARM. EE CLAIMED IT WOULD TAKE TOO MUCH TIME TO LOTO TO
EMPLOYEE WAS ATTEMPTING TO CLEAN THE MUD OFF THE CONVEYOR TAKE UP PULLEY BY USING A 4 FOOT STEEL BAR. THE BAR WAS CAUGHT BY THE PULLEY PINCHING EMPLOYEE'S LEFT HAND BETWEEN THE BAR AND EDGE OF THE CATWALK.
EMPLOYEE CLAIMS BACK STRAIN FROM OPERATING A LOADER OVER POT HOLES AND BUMPS IN STOCKPILE AREA.
EE WAS WORKING ON WINDSHIELD WIPER MOTOR AND THETRAVEL ARM WAS ON DELAY AND THE ARM CAME AROUND AND CUT HIS HAND
EE STUCK HIS FINGER IN THE AIR VENT OF HIS LOADER TO SEE IF IT WAS OBSTRUCTED AND PUT HIS FINGERIN THE FAN.
EE DID NOT LOWER BOX ON 40TON HAUL UNIT, CAUSING IT TO WEDGE INTO THE CEILING IN THE UNDERGROUND MINE AT APPROX. 25 MPH.
LULL, HIGHLANDER MODEL 844B STRUCK EMPLOYEE RESULTING IN FRACTURE TO BOTH LEGS.
TWO EMPLOYEES LOCKED & TAGGED OUT #2 MINE CONVEYOR. THEY DID NOT CHECK TO BE CERTAIN IT WAS DEENERGIZED. THE CRUSHER OPERATOR, UNAWARE THEY WERE WORKING ON #2 TAKE-UP PULLEY, WENT THROUGH THE START-UP PROCEDURE & STARTED BELT #2 THAT HAD A DISCONNECT SWITCH THAT HAD BEEN TAMPERED WITH. ONE EMPLOYEE WAS PULLED TOWARD THE TAKE-UP BUT WAS PULLED TO SAFETY BY A CO-WORKER.
EE RAN INTO PILLAR WITH HAUL TRUCK HIT HIS HEAD ON CAB CAUSING LACERATION TO FOREHEAD RESULTIN G IN STITCHES.
BEING RAISED IN MAN BASKET TO INSPECT CONVEYOR. FORKLIFT MAST CAUGHT CONVEYOR FRAME. CONVEYOR SLIPPED, PINCHING FINGER AND THUMB. NO PERMANENT DAMAGE.
EMPLOYEE CLEARING JAW JAM WAS KNOCKED DOWN ACCIDENTLY WHEN BELT WAS STARTED ACCIDENTLY. THIS INCIDENT HAPPENED ON 7/10. ON 7/22 HE WAS PUT OUT OF WORK BY THE DOCTOR FOR CONTINUING NECK PAIN.
COVER ON FAN IN LOADER CAME LOOSE. EE WAS LOOKING FOR THE SCREW IN THE FAN FRAME. THE FAN WAS RUNNING. HE PUT HIS HAND IN THE BLADE, CUTTING HIS RING FINGER ON THE LEFT HAND. 5 STITCHES TO CLO SE CUT.
EE WAS OPERATING HAUL TRUCK WHEN HE JARRED HIS BACK.
EE OPENED SIDE DOOR ON ENGINE COMPARTMENT OF CAT LOADER TO CHECK ENGINE OIL. APPARENTLY EE HAD NOT OPENED THIS DOOR COMPLETELY AND THE SHOCK ABSORBING PROP COULD NOT HOLD IT OPEN. THE DOOR FELL CLOSED, STRIKING THE EE IN THE BACK OFTHE HEAD, FORCING HIS FACE INTO ANOTHER ACCESS DOOR AND CAUSING FACIAL LACERATIONS.
EE WAS OPERATING WELDER, DIGGING IN A MUCK PILE. HE STATED HIS BACK HURT DUE TO JARRING.
EMPLOYEE ALEDGES HE WAS SHOVELING UNDER OR AROUND A HAMMER MILL WHEN A ROCK CAME OFF THE FLUX BELT CONVEYOR AND HIT HIM IN THE ELBOW AREAOF HIS RIGHT ARM. HIS ARM BECAME PAINFUL TO WHERE HE CAME TO MANAGEMENT SEEKING MEDICAL ATTN11/19/01. HE WAS SEEN BY A DR. AND SWELLING IN HIS ELBOW WAS DRAINED. HE RETURNED TO WORK WITH NO LOST TIME OR RESTRICTIONS.
EE WAS OPERATING A SMALL UTILITY LOADER WHEN HIT A DIP INT HE ROAD WAY JARRING HIS LOWER BACK. HAPPEN ON THE 16TH BUT DID NOT GO TO THE CLINIC UNTIL THE 17TH PER EE.
EMPLOYEE WAS OPERATING HAUL TRUCK ACROSS THE YARD WHEN HE HIT A HOLE THAT CAUSED THE MACHINE TO BOUNCE AND JARRED THE EMPLOYEE'S BACK.

EE WAS JARRED WHEN ROCK WAS DUMPED INTO THE BED OF HIS TRUCK. THE DOCTOR SAYS HE HAS STRAINED MUSCLE. THE DOCTOR EXPECTS FULL RECOVERY.
TWO EMPLOYEES WERE GETTING READY TO COME DOWN INTHE MANLIFT FOR A BREAK. ONE WAS HOLDING ON TO THE HANDRAIL WITH BOTH HANDS. WHEN THE OTHER WENTTO MOVE THE MANLIFT TO THE RIGHT, HE WENT TO THE LEFT INSTEAD, SMASHING THE INJURED'S RIGHT RING FINGER BETWEEN THE MANLIFT CAGE & CONCRETE PILLER. AS A RESULT HE NEEDED 7 STITCHES.
WHILE DUMPING A LOAD OF MATERIAL THE LOAD SHIFTED JARRING THE VEHICLE. THE JARRING ACTION OF THEVEHICLE HURT THE EE'S BACK.
