EE was struck by belt from behind causing EE to come in contact with belt rolling frame.

Employee was operating a 992K loader while loading customer trucks when EE heard a "pop" in EE's right wrist while using the control handles.

The employee was operating a WA600 loader and, while moving backward to forward on level ground, experienced neck & shoulder pain. This became reportable on 12/30/2025 after the employee was placed on restrictions following an orthopedic evaluation.

It appears that an employee experienced a personal medical event while operating a haul truck. This medical event is believed to have resulted in a loss of consciousness, leading to a loss of vehicle control and a rollover incident. Available evidence suggests that it may be attributable to a personal medical condition rather than factors related to the work environment.

While operating a loader in the pit, the individual was jostled in the seat and felt pain in their back.

Employee contacted the left elbow on the windowsill of the haul truck while getting loaded by a loader. Determined to be reportable on 12/04/2024.

8/6/2024, EE experienced unspecified back pain while operating haul truck. 8/7/2024, EE was seen at clinic, first aid only. 8/22/2024, EE condition did not improve, occupational doctor prescribed physical therapy.

Employee hit air line bracket when man-lift abruptly stopped.

Employee was loading structural fill with CAT 982 loader, dust got into eye because the cab door was not completely closed. Employee was taken to the local clinic, diagnosed with infection of the left eye.

Employee was shoveling around the locked out tail pulley. EE climbed between the trusses of the conveyor and shoveled toward the tail pulley. The belt rolled backwards and EE placed both arms toward the tail pully, placing arms between the tail pulley and belt. The belt continued to roll backwards pinning EE's arms between the tail pulley and belt.

Employee was operating a loader and was jarred by falling material.

Employee operating a boat contacted a sand bar throwing EE forward into a seat. This resulted in three fractures in EE's back.

A haul truck was being loaded in pit. The muck pile began to slough causing wall material behind the pile to shift. This material slid down onto and covered the rear of the truck enough that the operator was unable to drive away. EE safely dismounted the truck to meet the loader operator on the ground. Three days later reported had back pain and sought medical care.

Truck Service vehicle lost brake control due to unknown circumstance. Unable to brake causing impact to a rib.

The employee was pulling down on a belt which was connected to a motor pulley and a gear box pulley. The purpose of the task was to rotate the screw conveyor so bolts that were damaged could be replaced. After doing so, the pulley began to turn catching and injuring the employee.

Employee received a whiplash type of back injury when EE's loader contacted a haul truck when both were backing up. The incident became reportable on December 9th when the injury was determined to be work-related and the employee was placed on restrictions/physical therapy by our company physician.

While installing a metal collection box via use of a forklift to position in place and a ratchet strap being used to align bolt holes, employee placed hand on flange of metal box to brace self while releasing the ratchet strap. As the collection box made contact with the existing structure, the employees index finger became caught between the bolt flanges causing injury.

While operating the scaler, employee stated that the wrist was injured due to jarring the machine. Following an investigation, with company management and our work comp provider, it became reportable on 8-13-21.

Pit Loader bucket made contact with a rock and jarred the operator.

While lifting crusher liner, the hooked chain slipped off of the fork. This caused the liner to roll, resulting in the injury (bruising) of the employee's right leg.

Employee was operating a scaler underground. The scaler pick broke free dropping the scaler boom and jarring EE's back. Became a "Restricted Duty" injury 8-13-20.

Haul Truck Driver was driving to and from the cut to the dump. Did not report that they had injured themselves until the next day when they said they were having back pain.

Employee was operating yard loader. As EE traveled across the yard the employee hit a bump, which resulted in pain to the lower back.

EE was tasked with operating a 980G loader to build a new berm to allow drilling and blasting crews access to set up a new shot. The EE cleared material and opened the area to an exposed highwall edge, then backed up, towards the edge of the highwall, causing the machine to fall to the bench below, sustaining injuries from the impact.

Employee was tramming a ManLift and hit a bump forcing employee to strike against the basket.

Miner was using a hand controlled loader/skidsteer to clean surge tunnel. Miner's finger became caught between the controls and the frame of the conveyor causing a laceration and compound fracture.

Employee states that on or about July 12th, 2019 employee was operating a loader with a bad left armrest that caused injury to arm. On November 21st employee saw a doctor who gave off duty restriction and MRI as a followup due to work related injury. MRI shows torn rotator cuff.

At approximately 9:35AM employee was operating a tandem axel truck stockpiling material. Employee parked on a slope and began to dump. The box tilted causing the truck to fall onto its left side. The employee was able to exit the truck on their own. Employee experienced pain when walking so was taken to the ER for physician evaluation.

At approximately 8 A.M, an EE was starting conveyor 27. EE was standing to the side as ee was starting the conveyor. The panel energy discharged causing the panel door to open. The discharge of energy caused particles to come into contact with the EE. Particles from the discharge scratched the EE's right eye. The employee was wearing eye protection.

Employee was scraping out the side pocket box on Vacuum Truck. The mechanism holding up door failed causing the door to fall and strike the employee's right hand.

Loader was loading stone into a truck. Stone rolled out of bucket into side board rocking the truck. Operator jarred shoulder.

EE was operating a 272 CAT skid loader to remove snow from customer scales and contacted hard hat on the inside of the cab, resulting in the hard hat contacting above left eye. This caused a laceration requiring stitches.

Employee was setting the park brake on a JCB 509-42 telescoping forklift, upon exiting the machine EE noticed a strange feeling in the left bicep and that it was swollen.

While hauling rock to the crusher EE contacted the rock wall in front of them. This resulted in surface lacerations to their hand and face. EE received stitches and no further treatment/no restrictions.

A Volvo A40G off road 6x6 truck made contact with a Komatsu PC360 excavator while approaching from the blindside of the excavator. This incident was investigated by Mine Safety and Health Inspector on 09/12/2016. The complaint ID is 966940. We were notified on 10/03/2016 that the operator of the Volvo 6x6 truck had filed a claim of injury.

The employee was operating a Genie S-60 Manlift. While navigating a downgrade, employee drove over a small pile of rocks causing the manlift to shift, jolting the operator. When the operator shifted, they planted their leg causing a hairline fracture.

Operator was hauling a load of spoil material to the dump area, drove into a hole and right leg contacted the bottom edge of dash resulting in a small laceration just below the knee about 1/2 inch long requiring one stitch. Employee was treated and given a return to work release.

While scaling, EE was jarred in the seat when the rock the EE was scaling broke free. EE began having pain in back and reported it to supervisor. Resulted in a back strain. No deficiencies found with seat.

Employee veered into the face with haul truck causing the steering wheel to jerk back and forth. This caused a hairline fracture in EE's wrist.

Employee was standing next to a conveyor when a rock fell from the conveyor striking his left hand causing a laceration.

Two trespassers were operating dirt bikes inside the Martin Marietta quarry. The men were driving down one of the pathways, one of them collided with a closed gate, causing fatal injuries. The other person stated that they had driven down Old Charlotte Hwy and crossed over the rail road tracks to get inside the property. This fatal should be Non-Chargeable to the mine.

The employee was driving a man lift to another area in the plant when she hit a pot hole in the roadway that caused her to be pitched upward. When she came down and landed, she rolled and injured her foot and ankle.

Employee alleges that the brakes locked up in the Komatsu WA 500 loader he was operating injuring his lower back. He was seen at a clinic on 5/8/2015 and released to duty. Employee went to a specialist on 5/19/2015 and was taken out of work.

The employee was operating a loader and while going into a stone pile to load his bucket, the stone abruptly stopped the loader and caused his neck to pop out of place. This was medical only until he went for licensed massage therapy last week (May 18-22, 2015).

Employee strained neck by turning head to look behind him while backing up in his haul truck. PT, Prescriptions, and Restricted Duty occurred. At the time of the incident the mine operator did not believe the incident to be work related. We are now reporting as the incident has been accepted by Worker's Compensation.

Trespassers were operating an ATV near or on mine property. The ATV overturned and as a result we were notified that one of the trespassers was killed.
