Mostrando 50 de 742Employee was putting oil in a banana screen. There was a panel removed from the screen for service. The employee stepped in the hole created by the missing panel and fell and fractured three ribs.
Pumper was spotting the water truck under the temporary horses head on the lake bottom dump. Pumper climbed on top of the berm for a better view. As pumper was coming down off the berm, miss stepped and rolled ankle causing a sprain to ankle.
Employee was taking down a piece of tubing. When it came down it struck the ladder EE was standing on causing EE to fall off the ladder and EE hit left arm on the tubing resulting in a strain to left shoulder.
Technician slipped off boarding ladder. Technician's face contacted ladder causing a tooth to become dislodged. Tech was taken to the ER where the tooth was reinserted and the employee returned to work the next day. Employee will see dentist for additional work to fix tooth.

Employee was climbing up the steps on the 980H loader. EE reached for the door handle, the door handle was slick from the rain causing hand to slip off the handle as the door opened. This caused ee to fall off the loader hurting back, shoulder, neck and arm.
Employee was stepping out of a locomotive when they felt pain in their lower back. This is a recurrence of an injury suffered at Emerald Mine (3605466) on 5-20-2010. The employee has been treated for lower back pain since then and was taken back off work by their Doctor.
Employee was mounting D10R Dozer from right side. EE laid lunch bucket on track, grabbed handrail and when EE raised up EE's left leg to get on the step of the Pusharm employees right knee gave out.
Employee slipped while walking over a coal pile and felt pain in knee. Employee is a weekend warrior and first missed shift was 05/28/2016.
EE was doing 3:00 am rounds when coming down a set stairs at the plant EE tripped and fell down. EE was complaining of back, neck and head was hurting. Ambulance transported EE to Welch Community Hospital at 0423. EE was released was told to stay off work till 5-24-16. EE went to family doctor on 5-26-16 and was put off till 6-3-16.
Employee was walking from the intersection of #6 to #7 entry when ee slipped and tried to catch self causing ee to twist knee. The employee worked until 7-5-2016 at this time ee was taking off from work.
Employee was helping load a piece of belt and fell landing on SCSR. EE went to the doctor on 05-03-16 and was given a full release to return to work but was still having pain and went back to the doctor on 06-13-2016 is off work due to lumbar strain.

Employee was entering a scoop canopy. When EE stepped into the deck, right foot slipped causing EE to twist right knee. EE finished shift and sought medical treatment later that day. EE visited LRMC ER and was referred to Ortho doctor on Monday. Ortho doctor has excused employee from work through 07/02/2016. EE remains away from work.
While traveling to the section, The roof bolt operator exited the man trip to open the doors on the track and twisted right ankle.
Employee was working on a loft step ladder. EE went up the ladder to grind a bevel. EE got to the third step and the extension cord that EE had for the grinder was short. As EE turned to pull on the extension cord to get more slack, feet slipped on ladder rung causing EE to hit side on top of ladder.

The employee was closing a side pan on a D10 Dozer and it slid out of the hinge causing the employee to slip and fall off the Dozer. This was originally a record only accident but the employee is now receiving Physical Therapy for neck and back pain and is being reported as a Medical Treatment accident.
Employee was walking out of Prep Plant and stepped in a drainage ditch and sprained EE's right ankle.

As EE re-positioned the bar, the roller moved getting the employee off balance resulting in the employee falling to the ground. At the time of the incident, the employee felt fine. EE completed the shift and worked the next scheduled shift.
Injured was walking to the warehouse when EE tripped on a down drill that had been covered by the recent snow fall.
After parking shuttle car, employee walked to the power center and stepped in a mud hole, twisting employee's left knee. Employee was not given time off of work until 2-4-16.
A clean coal truck pulled onto the scales, employee observed the truck bed was on fire. EE grabbed a nearby fire extinguisher and while going around the mechanics truck EE slipped in the snow catching self before falling. Operator was informed on 3/24/2016 that employee has a testicular hernia and will have surgery on 3/31/2016.
Employee was walking out by the miner when EE's legs got tangled up in the miner cable causing EE to twist EE's left knee as EE fell. This resulted in a fracture in EE's knee cap.
The injured was walking across mine parking lot and slipped on a patch of ice causing a sprain right knee. The injured's first missed day of work was 1/15/2016
Employee was cleaning the tipple when the metal detector shut the belt off. EE turned to go to the belt when EE placed EE's foot in some mud and EE twisted EE's knee. The area EE stepped in the mud was a small incline. EE had been scheduled for an MRI and management received notification on 2/2/2016 that EE will require surgery.

While working on a brattice, employee turned to get some cap wedges when he tripped over a plaster bucket. As he was falling his body made a twisting motion causing severe pain in his lower back continuing down to both of his feet.
At Surface mine. Installing 10oz.felt and 40 mill plastic felt for impoundment. He was pulling plastic down the slope and at the bottom of the slope he went down. Stepped on uneven ground which made him fall. fractured left tibia and fibula.
Employee was checking gate at reclamation site. A chain was attached to gate. He tripped on chain and fell. Sprained left ankle.

Accident was not reported, and employee continued to work until 11/02/2015 at which time employee experienced another incident with his knee while attempting to enter the deck of an underground forklift. His first lost day was 11/03/2015.

Employee was operating Fletcher walk thru roof bolter installing roof bolts in 4 right xcut. He had drilled hole in roof, inserted resin and started bolt into hole. Employee was climbing down from a ladder that was used to gain access to the top of a shovel bucket job. As he descended the ladder, it slipped out from underneath him. As he was falling he reached out and grabbed onto the shovel latch bar to catch himself from falling resulting in a dislocated left shoulder.
