Worker was welding in shop. They removed their welding hood after completing a weld and the slag popped and sent material into their eye. The material had to be removed by medical professionals and they were prescribed antibiotic and steroidal eye drops.

The Injured Person (IP) was cleaning out the Roto-packer and bent over to cut a partially empty 50 lbs bag to discard remaining contents, and as the IP bent over, EE felt a pop in EE's lower back (left), and the IP had to take a minute before standing up straight. IP reported the incident to the Lead Operator. IP was taken to Urgent Care.
A manual slide gate closed too quickly on a miners right hand. The impact caused a fracture and a laceration requiring 8 stitches.
Injured Person was replacing the classifier bearing assembly. The assembly became lodged due to alignment. The injured employee had hands underneath the assembly, attempting to correct the alignment. Subsequently, the assembly dislodged and fell onto the injured employee's hands. This incident resulted in a broken ring finger and lacerations to the ring and index fingers.
Employee was cleaning under Belt 111 in Crushing. EE was bent or crouched over to access the area under the belt. When EE stood up, EE lost EE's balance and fell into a section of angle iron support, striking EE's head on the angle iron, causing a laceration on EE's head.
Team member was clearing a bag jam on the palletizer in plant 1 packaging line. When lifting a 25KG bag off the conveyor the employee allegedly felt discomfort in the front part of EE's right shoulder.
Were replacing a bad sprocket following troubleshooting of a gearbox. Removed the 4 bolts from the mounting plate, chain slipped and motor & gearbox fell. The falling equipment impacted the IW left pinky finger, requiring stitches, antibiotics and light duty.
The employee was attempting to determine the cause of vibration alarms, and while feeling for vibration, their hand was able to enter the guarded area, which resulted in right pinky finger making contact with fan blades, resulting in the avulsion of the distal end of their finger. The employee did not lose the whole finger, but the flesh on the tip of their finger.
Pulling dump truck out of tailing shed. Got out to make sure the tailgate was latched. As EE walked to the rear of the truck EE's feet slipped out from under EE on the icy surface. When EE fell EE landed on EE's left shoulder.
Incident occurred while installing a new pressurized air system.
A compressed system was installed. While energizing with compressed air, an air leak occurred in the piping. The failure caused the piping and the receiver tank to collapse. While escaping the area, an employee was struck in the back by a manlift, causing them to fall. During the fall, they struck their head and sustained a head laceration that required stitches.
Operator was doing pre-shift on a D8N Dozer while closing engine inspection door they slipped and fell, they broke their wrist as a result of the fall.
Miner was checking oil on the D8 Bulldozer and they slipped on the tracks. Their foot went between the tracks and cab, and their ribs hit against the grab handle. They injured their ribs. 2 weeks later on 12/1/25, they sought medical care and were prescribed a muscle relaxant.
2 employees were working on a palletizer, but only one of them had it locked out. When the locked out employee was done, they asked the other employee if they were clear, and heard "yes". The other employee was still working, and when the machine was started, their right hand was caught, amputating the index finger, and degloving the middle finger.
Injured person was working on the hydraulic system of a D8 Bulldozer in the morning. Had to access the system below the floorboards of the equipment, and was working in an awkward position. Later in the day, they experienced pain in side/stomach and left work to seek medical care. At the hospital the employee was evaluated for back pain and prescribed hydrocodone.
Employees knee buckled while descending the stairs and employee fell and twisted their knee.
Affected employee was cleaning out #5 Mill feeder and injured EE's elbow (elbow contusion) in the process.
The Operator was loading a truck out of 8 silo. EE attempted to hold the hose and ladder simultaneously. Employee shoe slipped and gate sprung back into its original position. This resulted in a strained shoulder. EE was given instruction to take over the counter medication, and not to lift anything over than 10lbs.
Miner was switching railcars, drove a buggy between a car being pushed and another parked, and was struck, causing EE's leg to be entrapped between the dash and seat.
Employee was sick from being dehydrated.
Employees were unclogging a baghouse and a hard hat fell into the screw at the bottom of the baghouse. The hard had was drawn into the screw and stopped at the rotary valve. They workers locked out the screw and valve and attempted to retrieve the hard had. When the hard hat came out, there was stored energy in the valve and it turned and caught the workers fingers.
Worker was cleaning the plant floor and encountered a guard that had fallen from a head pulley on a conveyor. EE attempted to replace the guard and the active conveyor caught it. EE's hand was smashed between the guard and adjacent steel casing.
The employee was lifting an empty 55-gallon drum (~48 pounds). During this lift, the employee felt discomfort in left biceps.
The employee was in the process of loosening a bolt on a piece of machinery. While pulling back on the wrench to loosen the bolt, the wrench slipped off the bolt and hit the operator in the mouth, striking EE's upper right central incisor tooth causing a hairline fracture in the tooth.
Sunny, incident occurred indoors with full lighting. The injured party suffered burns to both hands (non-life threatening) while installing a ground inside an MCC. The ground wire recoiled due to line memory and rebounded once let go into the adjacent bus bar creating the arc.
The mine site was thawing out from a recent cold weather spell that allowed for the roads and pads to thaw. This created an increased instances of thick mud around the mine.
Employee was called out to install a hose for 10 silo. The clamp failed and EE had to come back out and install it again. During the second installation the hose was found in the car, full of clay. Another Employee helped EE pull the hose out of the car and this was when the employee felt a pain in the lower stomach.
Operator inspected the mill tank for plugage. During inspection EE found a plugged area around the discharge of the tank. EE then retrieved a cleaning tool from the milling area and began prying product away from the discharge area. While clearing the product away from the discharge, the tool slipped from EE's left hand and struck EE's right index finger
Maintenance employee directed the mixer operator to stand on a ladder and see if the valve was opening all the way prior to reassembling the pipe. Mixer operator put their middle finger into the pipe opening. The valve closed on the tip of the finger. The impact caused an avulsion of the workers left hand middle fingertip.
Employee had a reportable hearing loss in EE's right ear.
Employee was welding a piece of small metal plate to cover a hole in some ductwork. After tack welding the plate the employee raised welding hood up to adjust the welding rod. Employee then heard a noise and looked up and the metal plate broke off and fell striking the employee in left incisor tooth causing a chip.
Lab technician was sitting, entering data at a computer in the lab, when they received a spider bite to their lower right back. Bite developed a bad skin reaction consisting or redness, blistering, itching & pain.
Employee was exiting truck when EE rolled EE's right ankle while putting foot on ground.
While installing the bucket on a skid steer, the employee could not close the latch easily. EE utilized a pipe to gain leverage on the latch and was able to close it. After this, the pipe was jammed on the latch handle. When EE pulled the pipe from the latch, EE smashed EE's left thumb on the skid steers frame. The thumb has a laceration and an open fracture of the distal phalanx.
Employee states was bagging 50 lb paper bags, on the 8th row of the pallet when rotating to stack the bag when EE felt pain. The pain felt to have come from EE's back, to neck, to shoulder. Management wasn't aware that it was work related until employee brought a doctor's note on 7/1/24.
Vacuum Truck was being driven around a corner when the truck tipped over, landing on its passenger side in & against a berm. Resulted in a broken tibia and fibula in left leg.
Employee was cleaning out the #5 Mill feed bin with a push bar to clean the material that had plugged up inside the tank. When the employee pulled to remove the push bar employee's left index finger became caught between the push bar and a guard rail.
EE was crossing the grass going to the crude shed and struck right knee on a jersey barrier.
IP was climbing down a ladder, grabbed a swing gate for support, the swing gate broke loose and the IP lost their grip and fell less than 3 feet. The fall resulted in a sprain to the right foot and ankle.
Miner was completing pre-start inspection and as miner climbed up to the cab miner's hand slipped off of the hand hold and miner fell approximately 2' to the ground.
After recent rain, the employee was walking in the plant area and slipped on some wet diatomaceous earth. The employee fell from the slip and landed on right shoulder and back.
EE was attempting to clean the bagging machine spout out with metal rod. The rod struck the roto packer and kicked back resulting in a right hand laceration.
EE was installing a new 4 wide drive belt on the drive belt on the Dryer Air Supply Fan for the Furnace. Belt was on driven pulley and they were rotating non driven pulley to install belt. When the belt jumped into place EE finger was between the belt and pulley.
Miners working to clear out a clogged screw conveyor, miner didn't follow the company Lock out/tag out policy and caught their finger in the upstream screw causing an amputation of the left hand ring finger distal phalanges.
Employee fell on top of a bulk truck trailer while loading. The fall resulted in 2 rib fractures.
A maintenance supervisor was walking up the Kiln 4 upper product belt carrying a piece of chute to be installed and EE's right forearm contacted a piece of expanded metal which was in close proximity to the catwalk. This contact caused a laceration which required seven staples to close.
Employee was performing maintenance and kneeled on the ground to perform work. When they stood, they felt a pop in their right knee. Employee had initial pain, but it went away. During the night after the shift, their knee began to hurt. The following day they reported the incident and were sent to the doctor to be checked out.
Miner was attempting to retrieve a bag from conveyor.
Worker was attempting to weld a short length of pipe to the bottom of a bin. EE tack welded the pipe in place in preparation for the weld. The tack weld failed and the pipe fell and the employees thumb was impacted between the pipe and adjacent angle iron. The impact resulted in a fracture to the left thumb.
