WHILE WORKING ON D10N, EE SLIPPED AND FELL FROM BOARDING PLATFORM. HE DID NOT FEEL MEDICAL ATTN. WAS NECESSARY. ON MAY 2ND, EE WAS IN ALOT OF PAIN & WAS TAKEN FOR MEDICAL TREATMENT. LDW 5/2/04.

EE REPORTS WHILE PULLING CABLE FOR ELECTRIC SHOVEL WITH HOT STICK, HE LOST HIS FOOTING & FELL ON HIS KNEES.

WHILE STEPPING OUT OF LUBE TRUCK, EMPLOYEE STEPPED ON ROCK AND TWISTED KNEE. RETURNED TO WORK, HOWEVER AFTER SEEING SPECIALIST, WILL BE SCHEDULED FOR SURGERY. LDW 03/23/04.

Over-exertion in lifting objects

While moving a shovel tooth with another employee, employee felt pain in his lower back.

Caught in, under or between a moving and a stationary object

While removing a sheave from spare cyclone feed pump bearing assembly, the employee appeared to use right index finger to check for movement between the sheave and the shaft. The sheave moved unexpectedly, resulting in laceration.

ON MARCH 29, 2004 EMPLOYEE WAS ATTEMPTING TO ATTACH FUEL HOSE TO DOZER AND FELT PAIN IN STOMACH. ON JULY 26, 2004, EMPLOYEE HAD SURGERY FOR UMBILICAL HERNIA. WAS NOT ABLE TO RETURN TO WORK - BEGAN TO LOSE WORK TIME ON JULY 28, 2004.

Employee reports while attempting to pry the dead-bed liner over in order to be able to extract a belly liner, the prybar gave way causing his right fifth digit to be smashed between bar and a nut that was welded to the liner.

Struck by flying object

EE WAS IN PROCESS OF LOWERING MONORAIL S BEAM FROM CULING WITH COME-A-LONG. S BEAM RELEASED FROM ANCHOR BOLT, BEAM TURNED IN RIGGING SWUNG INTO EE. MOTION OF S BEAM PINNED EE HAND AGAINST EE CHEST, CAUSING SIMPLE FRACTURE.

Employee was removing a bolt when the socket slipped off the bolt and struck his left thumb causing a laceration.

The ee states that when he was getting into a drill cab he fell back onto a rock, or rocks. Was not reported until 11-8-04.

While working on with a chain hoist lever, employee's left arm struck side of launder resulting in a laceration.

EMPLOYEE REPORTS WHILE WALKING THROUGH THE TUNNEL, HE BEGAN TO SLIP AND CAUGHT HIMSELF, THEN FELT PAIN IN LOWER BACK.
