Over-exertion in pulling or pushing objects

On 9/4/17 employee felt sharp pain in abdomen. Employee informed supervisor about pain but it subsided and employee returned to work. On 9/6/17, the employee reported to supervisor that the employee started to feel the sharp pain again. The employee was then taken to the clinic to be examined.

Supervised Geo Temp employee working as a drill technician reported an eye irritation. Employee reported that the eye irritation was from dust.

Technician was replacing a step on the pusharm of a D10. Tech tack-welded it in place and went to weld the underside when the 100lb step fell off and landed on thumb.

While being loaded by Shovel 10, shovel hit the back of the bed causing the employee to feel pain in neck and shoulders

While picking up tools at the end of shift the employee while descending a few steps of a short stairway to had the tools to another employee missed a step and left leg rolled under ee as ee caught self with the handrail. The resulting injury was a broken fibula and tibia. Contributing factor may have been different rise height on the first and 2nd step

Employee reported that while operating Shovel (AC10), employee's right wrist began to hurt

Employee reported while being loaded by Shovel 8, a boulder hit the bed of the truck shaking it.

The employee was riding in a transport bus to EE's job assignment when the bus hit a bump in the haul road. The employee then reported to EE's supervisor that EE was feeling some back pain after the bus hit the bump. The employee was taken to the clinic to be examined and after the examination, the physician pulled EE off work until EE is able to see a specialist.

Struck against a moving object

Truck was padded with loose material for a boulder to be loaded. When shovel swung over the bed to load the boulder, the boulder fell through the bucket door onto the back of the haul truck bed. This caused the truck bed to pick up and fall back down jarring the truck driver.

Employee slipped on berm and fractured wrist.

Employee was attempting to remove an empty 55 gallon steel drum off an eight inch tall secondary containment pallet, slipped and fell onto left side.

Employee stopped forklift to pick up a piece of broken plywood that had nails to throw it away. Employee unbuckled seat belt and stepped forward with right foot and felt a loud pop in knee.

Employee went to dispose of pallet in dumpster at warehouse 42 and felt a pain in the right arm.

Operator was stepping off D10-22 for fuel when employee twisted employee's ankle dismounting.

Employee was installing tracks on CAT Track Dozer D10 - 27 on the right side. The tracks kinked and the employee attempted to un-kink it with a 12lbs hammer. As employee struck the track, fly-metal struck employee's right leg.

Operator was exiting cab of haul truck, stepped up on catwalk, leg came in contact with corner of AC door, causing a puncture wound.

Employee was laying on the floor using a needle scaler to clean up welding cuts, and got some debris in right eye.

Employee was helping place a cutting edge into the back of a pickup. The cutting edge slipped and smashed their finger between the cutting edge and the tail gate.

Employee received a laceration of the right forearm when a piece of banding steel that was being removed from an item sprung back and struck the employees forearm.

Hot stick slipped off rabbit box resulting in the handle of hot stick hitting employee on the bridge of the nose.

Employee was trying to install the hoist rope onto the drum and pinched finger between the rope and the becket holder.

Two employees were inside the Ball Mill, attempting to remove a ball chip that was lodged between two liners. A 4lb metal wedge was inserted, between the two liners, to try and hammer it out. One employee swung the hammer while the other stood back. Upon contact with the hammer the wedge bounced back and struck the employee in the mouth.

Struck against stationary object

EE lost control of broken piece of liner material dropping it, resulting in a laceration to right palm requiring stitches.

Employee reported potential seizure and initiated EMS response. Upon arrival EMS found the employee alert claiming they have had an anxiety attack. EMS transported the employee to emergency room for further evaluation.
