Drilling a hole in a J-box. As he was pulling the drill back, the bit caught in his glove and drilled through his hand
Employee tripped over a chain attached to a come-a-long and fell onto a rock with his knee.
Employee reports that a punch broke on an iron worker resulting in a splinter of steel going into his left hand.
Installing buss bars at the SX-EW plant, buss bars fell from the ceiling. We had questioned Phelps Dodge Engineer who said buss bar hangers were safe.
Employee went up a ladder to repair a lube oil on GL, when the wrench he was using fell and he yelled to the employee holding the ladder to watch out. Employee holding the ladder stepped out of the way of the wrench and the ladder slid out from underneath employee, causing him to fall 20 feet.
Employee was on his back tightening a hydraulic hose that was overhead when he strained his shoulder as he put pressure on the fitting.

While descending ladder off of Rubber Tire Dozer, employee lost footing and attempted to stabilize self, grabbing exhaust stack of RTD. This resulted in a second degree burn to left hand.

While working on a drill, employee tilted a ladder back and leaned it to the side. As the ladder moved, employee stepped sideways to catch it and stepped into the drill hole on the deck hitting his side of his chest.
