Inhalation of radiations, caustics, toxic and noxious substances

Employee was welding down a transformer on a shovel. After finishing the job, he felt sick. He did not report illness until Jan 2nd when sought medical treatment.

Fall from machine

Employee fell from the deck of a parked haul truck, while attempting to clean the driver's side mirror

Over-exertion in pulling or pushing objects

Employee was loosening a valve when it came apart causing injury to right shoulder. This was a first aid until March 26, 2013 - employee had surgery.

Employee slipped on muddy surface and felt pain in right knee. This was a first aid until December 3, 2012, employee got surgery on knee.

Employee was working on grader when his foot got caught between the pedals. When he went to sit down, his right knee got twisted, causing pain to the knee.

Struck by flying object

While removing a bit from a sub with a hammer, a small piece of metal was lodged into employee's right arm. Doctor recommended leaving it, but employee got second opinion. Became lost time incident on 07/27/12 due to surgery to remove metal.

Struck against a moving object

A haul truck operator was backing under shovel bucket, when the bucket hit the bed of the truck causing pain to the driver's back.

Struck against stationary object

While installing liners EE's loose control of liner and it falls making contact with EE's right thumb resulting in the thumb being severed.

Struck by falling object

Employee reported that while trying to move a window mount a/c unit to the shelves below, the straps on the box broke causing the a/c unit to fall and hit employee in the knee.

Employee was pulling himself into Mack truck when he felt pain in his left shoulder. Employee was sent for an MRI and did not miss any time from work until 3/19/2012 when it was determined is was a tear in his shoulder.

While signaling a forklift to move a crate to a shelf in the connex, the crate got stuck against the shelf support and the employee reached in and pulled on it, pinching the left thumb.

Employee climbed up on dirt mound, as he was climbing down, felt pain in right calf muscle.

EE was removing a piece of stuck core from core shore. Sustained puncture wound to L hand.

While cutting open bag, moly dropped forcing both hands down, causing knife to cut left hand.

Employee reports that while performing repairs on support equipment, he pulled out the top drawer of his tool box resulting in the box toppling over causing the employee to be pinned between box and vehicle being worked on. This caused a laceration to his left lower arm

Welding overhead installing D-Rings on HT#125 a piece of slag fell and caught shirt of left arm on fire. Due to leathers not on the left arm received a 2nd degree burn from distal third of upper arm to distal third of lower arm. Doctor at Bagdad Clinic treated as a first aid by bandaging the burn and releasing him back work that day with limitations use of that arm for the day

Struck by falling object

Employee was pushing air line up side of frame of haul truck when the air hose came down, striking employee on the mouth, chipping tooth.

Employee was backing up a pallet mover when his left index finger got caught between handle of pallet mover and pallet rack resulting in a laceration.

Employee was throwing out an old table when his middle finger on right hand was caught between table and dumpster causing a fracture to the tip of his finger.
