SH8 Operator sustained rough loading injury after hitting a hard spot. The seat was discovered to be damaged allowing forward and back movement of about 3-4 inches.

HT205 Operator reported back pain from rough load at SH11. Shift started at 6:30 PM on 10/12/2023.

Haul Truck Operator reported that EE's lunch bag shifted as EE was getting into the Man Van at the beginning of shift and aggravated an old back injury. EE took pain medicine and EE's back was feeling okay. After the medicine wore off during the shift EE's back started hurting. NOTE: Reported back pain at 2:45 AM on 03/09/2023; shift began at 6:30 PM on 03/08/2023.

Mill Ops Employee on the way out of the mine site lost control of motorcycle when deer jumped in front of them.

Employee walking the berm on the Tailings dam and digging out cyclones slipped on wet slimes, fell and injured shoulder.

HT153 was rough loaded at SH12 causing injury to the HT Operator and the passenger.

CLP Employee working near the vent valve at the big boiler felt a splash on their face, wiped their face with their gloves on and felt burning.

Dozer D11-4 Operator slipped on muddy step and fell on push arm, causing injured ribs.

Employee tripped over a lifting eye welded to the crusher doughnut and fell, sustaining injury. NOTE: Shift began on 02/22/2023 at 5:30 PM; incident occurred on 04/23/2023 at 4:30 AM.

Employee burned their hand on the bottom of a glass coffee carafe.

Employee was walking/looking down at hand-held device and tripped over forks of forklift. EE was evaluated by WVFD and made no claim of injury. On 4/4/2023 EE reported swelling of right knee and pain in left shoulder since the incident date and was seen at the clinic and given restrictions. EE has not yet been released but resigned on 4/4/2023.

Employee flushing water through a caustic line was sprayed in the face when they disconnected the hose.

Employee rolled ankle stepping down from concrete pad to dirt. Please note that Employee's shift began on 02/15/2023 at 6:50 PM and the incident occurred on 02/16/2023 at 2:40 AM.

Struck by falling object

While changing forklift operators during suspended load operation, one of the operators inadvertently pressed the park break release switch while grabbing a hold of the grab handle when descending - causing the forklift to roll and the load to shift. Injured employee was caught in between pipe flanges - resulting to an injury to their wrist and thumb.

Employee reported eye irritation from debris that fell behind their safety glasses.

An MPC driver attempted to connect EE's 3" off-loading hose to an above-ground tank when it slipped out of EE's hand and came back, hitting the driver in the face.

Employee exposed to H2S gas on 7/5/2023 reported feeling ill on 7/11/2023.
