Accident type, without injuries

Lightning caused fire in remote portion of property. There was no possible danger to employees or processes. Due to remoteness and danger of trying to reach fire, decision was made to let fire burn over night. It was extinguished the next morning 07/04/11.

Fall onto or against objects

Employee was descending down stairs of haul truck when her right foot caught the fourth step, causing her to fall back against the steps and sit on her ankle.

Over-exertion in lifting objects

Employee reported that he felt pain in his back while disconnecting feed lines. This was not reported until now due to complete investigation of claim.

Struck against a moving object

Employee was cleaning up rocks with rubber tire dozer when the blade caught on a large rock, causing employee to jar sideways.

Contact with electrical current

Employee was installing switchgear bucket into cubicle, when he received an electrical shock.

Over-exertion (Not Elsewhere Classified)

Employee was carrying a liner chute when he stumbled causing pain to right knee. This was a first aid incident until medical review determined he needed surgery. This became recordable on 4/4/11.

Fall from ladders

Employee reported that while putting away stock, she missed the last step of a step ladder she was using. She pulled the ladder down with her and fell on her back and jammed her right thumb.

Employee reports while climbing down last step of ladder, he felt pain in his knee. He did not report this until the next shift. Employee worked another shift then missed two days. This incident was not submitted until today due to investigation of occurrence.

Struck by falling object

An employee was prying worn liners off the feeder wall when the liner above fell on another employee's arm.

Struck by flying object

Employee was using a 12# hammer to strike liners for removal in the ball mill upon striking a liner a piece of metal frayed and stuck employee in right leg. The metal went undetected until 10/18/2011

Fall from machine

Driller was arranging drill rod on rod truck as it was being loaded and fell off the end of the rods.

Employee knowingly & willfully circumvented health & safety policies by climbing over a handrail and walking across a dam wall. EE fell off wall and landed on his left side. He was checked out and released back to work. During his follow up the PA-C requested x-rays, which were positive for cracks in ribs.

Over-exertion in lifting objects

The driller thinks he may have strained his back while he and his assistant were placing timbers under the drill. The incident was not reported until the following Monday 05/23/2011.

Employee was tightening a 2 foot section of scaffold pole to an I beam for a brace. As he was tightening the clamp at the end of the pole the clamp in the middle came loose and caused the other end to cantilever and hit him above the left eye requiring sutures to close the wound.

While grinding, with safety glasses and face shield, wind blew dust in left eye. Eye washed. Taken to clinic to check eye, eye washed again, particle removed. Salve given for small scratch.

Employee was examining ac motor on haul truck, as he was doing this, his finger made contact with fan motor, resulting in a laceration to his right index finger

Employee reported he was using a torch to cut a rusted bolt. The bolt popped, sending debris into employee's right eye.

Employee reports after spending the day welding and grinding, he felt irritation to his eye. He did not report the incident until 7/14/11.

While replacing cylinder on crusher, employee's 5th left finger was pinched between cylinder and handrail.

Break out wrench lost bite of drill rod and kicked back in driller assistant's hand before regaining bite. The employee's wrist was originally examined and first aid care was administered. On 7/21/2011 an MRI showed a minor fracture of wrist bone and torn cartilage.
