Accident type, without injuries

Contractor entered elevator when he heard a loud noise when the doors shut. Elevator became stuck and he called operator line.

Employee was trying to open a drawer when she felt pain to her lower back

While employee was going down stairs, he slipped and fell, hitting left knee.

Employee reported while adjusting a coolant cap on a de-watering pump, coolant sprayed him in the face and arm.

Employee reported his right shoulder was in pain after installing a liner - this became a lost time incident on 07/08/2015 after medical re-evaluation

Fall to the walkway or working surface

While performing pre-shift inspection, employee slipped and fell in parking ditch.

Employee went to step off grader to get fuel. When he placed his right leg onto the bottom rung, his right leg twisted causing pain to leg/knee.

Struck by flying object

Employee was unloading a pneumatic trailer in a silo. While he was off loading the product, hose became plugged up. He released the tank pressure & began to remove the product hose. When he opened the cam locks the pressure blew the hose & employee back into his trailer causing a laceration to his face, a bruising to his left knee, arm and back.

Over-exertion in lifting objects

Employee reported feeling pain in right shoulder and hip after lifting a 16 lb hammer. This was not reported immediately due to an investigation of the incident. MRI was scheduled for 12/03/15 - results have not been received.

While trying to open haul truck door, employee felt pain to left shoulder

Fall from machine

Employee fell off the back of flatbed truck while trying to fix a truck tarp, hurting his right shoulder.

Struck by falling object

The helper was cutting the tape away and they believe the cable on the spool was over lapped and it dropped about 6 inches pinching his finger between the bail and the casing.

Struck against stationary object

Employee was stepping down from a ladder and hit his left elbow against a very sharp edge that caused a laceration.

While using a cordless drill, employee drilled through plate and struck his right index finger.

While opening knife blade on multi-tool, employee sustained a cut to his right thumb

Employee was coming down stairs when he slipped causing his left shin to hit staircase, resulting in a laceration.
