Employee had finished removing scaffold and was climbing down from the building structure. Employee lost balance and fell approximately 8 feet to grade. The employee suffered a fracture of the left tibia and fibula.

while sitting in buddy seat of haul truck, a large rock went over the teeth of bucket and landed on the front right corner of the canopy causing employee to jar hurting EE's back
While looking at the spare CFP, employee knelt down to check the sheave and felt knee cap pop out of socket. Employee popped it back into place and continued to work. Later it began to swell. Employee informed supervisor at this time. Employee did not go to the Clinic for treatment until 8/5/2019
While cutting tubing for hand rail, grinder bound and kicked back causing a cut to employee's leg.
Employee had just finished connecting a steel deck support. Employee tool belt got hooked by the rigging and employee was lifted about 6 feet above the deck. The tool belt broke and the employee fell to the deck structure injuring EE's right shoulder, right arm and right side of torso.
While climbing ladder on Rubber Tire Dozer, employee lost footing with left foot, then EE's left hand slipped off leaving two points of contact. EE swung to right side with right arm being extended and felt the elbow pop causing pain.
While walking from grind line 2 to grind line 1, employee slipped on wet concrete and fell hurting ankle
After the master pad bolts and pad were removed the track was still connected. Lead Tech went to get a pry bar when other EE put left hand on track and kicked the master link to free track. EE had finger between track pads and was pinched upon release
Employee turned ankle while walking to personal vehicle in the Mill parking lot.
Employee reported that while sitting down in AC-10 drill, the steel door on the shovel opened due to the movement of the drill. Employee reached over with left arm to catch the door as it was closing. Employee felt pop in shoulder but did not report it until the next day. Employee saw Dr. again 10/03/19 where employee received restrictions.
While reinstalling fuel tank, employee inadvertently placed hand on forklift fork supporting tank. Forklift operator shifted forks for positioning and pinched employee's right finger between tank and fork
While driving haul truck 205, employee hit a bump in the haul road and immediately felt abdominal pain.
While stepping off stairway onto uneven ground employee rolled ankle causing injury to right ankle
While installing Mill insert on ball mill 3 it became jammed. While trying to maneuver the component for final placement, the insert popped out of place, the come along tools assisting with the job failed sending the component into a free spin striking 4 employees.
Teammate was using a 1" impact gun to install lugs on wheel and lacerated left thumb
While inspecting air conditioner on Haul Truck 209, employee locked out transmission and left truck running. Employee slipped while using a ladder and EE's left pinkie finger slid between the alternator guarding contacting the alternator cooling fan causing a laceration/fracture
While operating haul truck from N3 to BD4000 dump, operator reported hitting bumps in the road and also that the bed of haul truck slammed down during 2 loads. This caused them to jar and experience lower back pain.
While attempting to turn off a 2" overhead valve, employee stepped up on a pipe and felt a pop in EE's left knee causing pain
While closing the tailgate of an ATV, employee had to apply force and employee's knee buckled and employee fell to the ground.
Shovel bucket made contact with the dovetail of the haul truck rocking the truck causing employee to feel pain in back and neck.
While being loaded in Haul truck, a large boulder was in the first bucket. The truck bed was not padded and the hard load jolted the employee causing mid back pain
Haul truck was backing truck up to the berm. While the truck was lowering the bed, driver noticed the outside of cab was engulfed with flames coming from the engine area
EE WAS PUTTING AWAY TWO HAMMER WRENCHES THAT ARE USED TO HOLD BOLTS ON A FRONT SUSPENSION. EE KNEELED DOWN TO LAY WRENCHES ON THE BOTTOM SHELF OF ROLL CART AND RIGHT KNEE GAVE OUT.
While dumping a tilting trash hopper using a forklift, employee lifted hopper with forklift onto a rolloff and pinched left middle finger between fork and hopper base.

While using hand hydraulic ram to position rear hoist bushing, the ram slipped off the bushing and the pump spun and slammed to the floor with employee's hand in a down force motion causing a wrist pain
While removing a 793B Haul Truck differential and transmission in the shop, transmission and differential freed up from the differential housing and swung into employee hitting EE in the right knee.
While pulling up to get on dozer, employee reached for grab bar and felt sharp pain in their shoulder
While tack welding on rock knockers on a haul truck bed in the overhead position, a hot chunk of molten slag fell from the weld puddle and landed on employee's upper right thigh causing a burn.
While carrying a 5 gallon bottle of water up the stairs, employee collapsed and slid back down the stairs.
Mud fell from dove tail of haul landing between employee's safety glasses and eyes causing pain in left eye
While employee was drilling through 3" channel steel with a hand drill, the drill bit caught and spun the drill motor around hitting employee in the lip with the battery.
Employee was trying to secure 3" female camlock fitting to 3" male fitting. While holding the hose in left hand, employee opened the valve. Unexpected pressure on the hose caused the fittings to come apart and cut employee on the right forearm
Employee was using a Husky utility knife to remove a rubber boot that was stuck on the intake tube of a haul truck. While cutting on the boot, the knife slipped from the rubber and cut the EE's forearm. Coworkers helped EE wrap the wound. EE received 7 stitches.
