Struck against stationary object

Employee stepped on a plastic shield and slipped, hyperextending her right knee.

Over-exertion in pulling or pushing objects

Employee was pulling power cable by hand with a hot stick on level ground. While he was backing up pulling the cable, he felt his right knee pop causing pain. Employee was released back to work 8/20/08. He has since seen a specialist and had surgery 9/26/08.

Employee was stomping on a stem lock bag to activate the chemicals inside when he felt his right knee pop out of joint.

Fall onto or against objects

Employee was checking the cleaner circuit on B1, while walking on the catwalk between the second set of cleaner banks 1 and 2 tripped over a spray bar that had been left in the walkway leaning up against the cleaner bank 1.

Fall from machine

Employee was getting a ladder from bed of service truck, missed the step getting off the truck and fell to the ground landing on his hip and shoulder.

Employee was operating a loader when the pile of gravel came down and caused the back tires to come off the ground and bounce the operator inside the cab.

Employee was holding cable with a hot stick when the stick was pulled from his hands, causing him to lose his balance and stumble over a rock.

Employee was trouble shooting the mill water chlorinator when he inhaled some chlorine gas.

Employee was respotting at the shovel when the load from the shovel bucket fell onto the dove tail of the truck, causing the driver of the truck to be tossed inside the cab.

Employee was walking backwards while operating an overhead crane when she lost her balance. This caused the employee to fall and hurt her left wrist.

Water truck rolled off soft shoulder when driver got too close to the edge of road. Driver received three stitches to chin.

Employee was driving a haul truck and complained of lower back pain at the end of her shift.

Employee pulled open the decking on the drill, turned and fell through the hole on the decking, causing pain to his right shoulder.

Employee was stepping out of a Porta John when she twisted her ankle.

Contact with electrical current

Replacing the wire rope on 2-ton Maxwell, Manning and Moore hoist, removing twist from wire rope, he removed becket from electrical panel end of cable. Ran hoist up and down to relieve twist in wire rope. Ran hoist control from remote control box with e-stop. Fastened wire rope end anchors thru back of electrical panel near transformer. No mainline.

Employee states that while loading an empty core tube down the hole, he was using the palms of his hands to guide the tube down when the over shot released the quad latch assembly and he smashed his right ring finger and right pinky.

Employee was using a knife to open a bag of solka flok when his hand slide over the blade and cut his right thumb.

Employee grabbed the trip cable and pulled down to relieve the tension on the compound arm, the compound arm came down rolling the tear drop forward. Trip cable became free, pulling the excess slack into the shive. Employee was unable to let go of the trip cable soon enough to avoid his fingers being pulled into the shive of the tear drop.

Employee was leaving 3 belt XCC MCC room and twisted his ankle. He reported this to his supervisor when he returned to the shop. His supervisor asked him if he needed to go to the clinic. Employee declined going to the clinic, he claimed his ankle was sore but he was ok.

Employee reports while changing a bit, she put the bit box in the deck plate and smashed her right index finger.

Employee was moving a pump bowl flange. He had one hand on the table and with the other hand grabbed the pump bowl flange by the shoulder of the flange and was sliding the bowl flange around pinching the flange against his steel toe boot and the bowl flange.

Employee was loading barrels when he moved a drum, it got hooked on a bolt from the bucket and slipped back against loader bucket pinching right 5th finger resulting in a laceration.

Employee was arc welding at work, went home woke up with eyes hurting. when to hospital. Doctor diagnosis it as flash burn
