While walking back to passenger van, employee twisted left ankle. This incident did not become recordable until 01/30/07 due review from medical.
While pulling cable, employee felt pain in lower right leg.
While arc-cutting sticks on shovel, employee's pant leg caught on fire, burning his lower part of his right leg.

Employee was stripping starter sheets when he hit his right elbow against hanger bar.
EE was removing brush from a hillside, lost her footing and slid down the embankment. The fall twisted her right leg and hip.
While stepping on copper starter sheet, his left foot slipped causing his left knee to twist.
Dismounting off drill and got to the last step and lost her footing and fell to the ground.
While putting a sling on a liner, the liner shifted striking employee on upper right thigh.
Employee was moving cribbing during truck disassembly at dump site. Cribbing needed to be moved numerous times in order to make crane pick from dump area to tranport truck. Cribbing pieces weigh in excess of 100 lbs per piece. Approximately 50 pieces of cribbing needed to be moved during this process.
Employee was moving fully loaded 55 gal drums from the lowere grinder floor. Employee was trying to roll the drum from a tight area and dropped the drum on his foot. The drum hit his foot on the area above his steel toe.
Ee was in a trench installing a choaker around a pipe and felt a sharp pain in his pelvic area. EE did not report it immediately, went home that night, then reported it the next day, 9/13/06.
While walking on wet floor of basement, employee slipped and struck his right knee on grating.
While backing under shovel dipper, dipper came in contact with haul truck bed causing cab to move, resulting in pain to employee's neck.
Employee was cleaning around conveyor system when personell from a different company began cleaning off area above here. A rock thrown by other company from above Meridian employee struck her in the shoulder.

Employee was cleaning up lime spill when lime contacted her right calf resulting in irritation. Three days later the irritation became infected.
While attempting to push a spinning bolt back into position, bolt cut employee's left hand.
Employee & partners were atemplting to reattach ball mill feed chute to feed cart. They were having to shake the feed chute to try to align with feed cart when feed chute broke loose from being hung up it pinched employees hand between feed chute and feed cart cauing a cut requiring 3 stitches.
While operating rubber tire dozer over rough terrain, the seat collapsed resulting in pain to lower back.
Employee and partner were removing pillow block bearing from pulley assy. Injured employee was holding housing bolts from spinning from bottom of bearing assy. After bolts had been loosened and nut removed partner began striking housing with hammer after second strike with hammer bearing separated and pinched injured mans hand between floor and lower bearing half.
Employee was bitten by unknown insect resulting in allergic reaction.
Employee was stung by a bee causing an allergic reaction.
While operating rubber tire dozer, employee was bounced around in cab due to rough terrain and hit side of cab causing laceration to right ear.
While installing an air compressor on haul truck, compressor slipped from mechanic's grip and caught the tip of his left index finger between compressor and engine cross-beam.
While using a torch to cut bolts, slag blew back on employee's wrist and forearm.
While loading under a 994 loader, the first bucket was put into the truck causing it to bounce up and down. Employee felt pain in his back a couple hours later. It was not turned into the supervisor until the end of the shift. Details from medical review were not turned into PDBI until 1/23/06.
Employee became overheated and dehydrated. Employee was seen drinking large amounts of gatoraide. Employee went to doctor and was told to come back to full duty the next day.
While working for a few hours, employee began to feel pain in his chest. Employee was taken to medical facility and it was determined he had heat exhaustion and dehydration.
