While employee was getting off a haul truck EE came down to the lower stairs. EE bent over to pick up an item and became dizzy upon standing. EE fell forward off the haul truck onto EE's head and right arm.

While fueling haul truck, employee stepped into the parking ditch for the left front tire and fell to the ground. Employee felt pain in right ankle, left knee and left arm.
Driver swerved to avoid deer in road. Driver was speeding. Tractor/trailer rolled on its side and was towed. Driver was treated and released from hospital morning of accident with minor cut to head, bruised hip and sore muscles.
Employee bent to tie shoe and felt pain in their back.
Employee felt pain in right hip while walking on uneven ground.
While operating a haul truck, employee reported that the employee was bounced around from a rutted road and felt pain in employee's back
During shift change, man van was traveling down the ramp to drop haul truck drivers off at the S10 tie down, when man van hit a series of bumps causing the bus to bounce up and down causing passenger to feel pain in back
While pioneering a road, and due to road conditions, track dozer rolled over
Employee was operating a come-along to position a D11 dozer blade stabilizer into position to connect to the machine. Come-along was improperly rigged and the chain was on the safety latch and broke free and stabilizer fell onto employee's right foot.
As cable was being rolled up it became stuck in some mud and when it broke free the cable struck employee. Employee regained footing and while walking back to the vehicle started feeling pain. Employee called for supervisor and was taken to clinic for evaluation. The employee's leg was fractured and they were unable to return to work for next scheduled shift.
Employee reported that while descending haul truck ladder at tie down, knee buckled causing employee to fall
After sampling groundwater wells on 7/19, ee noticed a rash had formed on right ankle below the boot line. EE returned to work on 7/27. The rash on the right ankle was more severe at the end of the day. EE continued to have irritation through the week resulting in a doctor visit on 7/21 where ee was diagnosed with cellulitis by a medical professional.
While washing rock by 2-3 chute at the walkway on grind floor, employee took a step forward and slipped and fell landing on left side
After moving a porta john, employee was raising the tailgate on the truck and felt pain in their back.
While air arcing on feed cart, employee's clothes caught on fire.
While cutting belt, employee cut left thumb with razor knife.
While descending boarding ladder of haul truck 206, employee slipped and hit back against the ladder.
Operator stepped off of D10-22 on the SW4000 dump to talk to trainer on the ground. Operator had right hand on grip and right foot on the step as the left foot touched the ground. Operator stated they felt pain in their left foot upon this action.
Employee had tripped and fell on a rock while conversing with another employee during their fueling process.
Employee reported that while performing leach maintenance, employee tripped on a rock and fell injuring knee
While assisting with a rebuild on the secondary crusher, EE had EE's arms extended and felt a sharp pain on the right side of EE's rib area
Employee hurt ankle while pulling drip line on Kimberly 3950 leach field
While being trained on the roll off wrecker, employee's finger was caught between the roll off tow truck rear deck and the j-hook
While removing 2" nut from the feed end of the number 3 auto mill, impact wrench slipped and employee hurt left pinky finger.
Employee was pushing two batteries on a pallet and felt pain in the hip.
Employee experienced a rough load from Shovel causing neck to hurt
Shovel was loading HT 131 when the dipper contacted the bed causing employee to feel pain in neck.
Employee burnt the palm of EE's hand on a vice that was used to make brackets
Employee received scrapes on the back of EE's calves due to ill fitting boots. Moly trioxide came in contact with the scrapes causing a chemical burn
Employee injured left finger while setting cribs for side frame rollover
Employee reported that while driving their right eye started to burn and itch.
While installing belts, employee's finger got caught between the sheave and the belt.
While cutting an apple with a leatherman, employee cut right wrist.
While employee was changing teeth on Shovel 10, a nut came loose flipping back and hitting the employee in the lip.
An AECOM employee involved in a subsurface investigations for a prefeasibility study for a new tailing impoundment stepped on a rock as they exited a vehicle. This caused the ankle to twist. Employee later recieved treatment including a brace, prescription ibuprophen, work restrictions, and physical therapy. The injury did not reslut in lost time.
Employee reported that while picking up a rag off EE's tool box, a drill was on top of it. The drill fell and the employee tried to catch it resulting in a small puncture wound to EE's palm
While cleaning out the Kiln, employee got a foreign object in their eye.
Employee was washing on GL-5 lower grind floor when the high pressure water hose broke causing spray to go in face and eye
Employee was tightening bail pin split collar bolts when the ladder they were on slipped out from under them causing them to hit the shovel causing a laceration under the eye. This injury did not become recordable until an antibiotic was given for an infection of the laceration 03/17/2018.
Toward the end of the shift, employee began to experience symptoms of heat related illness.
