Employee was using a 3' line up bar to hold liners up through a bolt hole in feedcart. Another employee was using a forklift to push liners into place. One of the liners got wedged in the feedcart so the forklift operatior backed up to reposition. The liner slid down and hit the bar causing it to strike employee in the face.

While attempting to raise and attach exhaust to truck, pipe shifted and fell, striking employees left index finger resulting in a laceration.
While pushing shell liners into a ball mill, a liner made contact with the chute side causing it to roll back. EE tried to stop it from rolling back and his left index finger tip got caught between the roller deck and the liner. A laceration and fracture of the end of the finger bone resulted.
EE was removing a tag line to place, the pipe shifted smashing his lower left leg between the pipe and the existing grating causing a laceration resulting in 15 sutures.
While changing a hose, hose broke, spraying compressor oil onto face and eyes.
Employee was assisting with traffic when he stepped on a rock, twisting his left ankle.
Employee was holding a knocker wrench while another employee was swinging a hammer trying to remove nut on a spider. Employee holding the wrench was struck on the finger by the hammer.
Employee was squatting down opening boxes with a box cutter, when the box cutter slipped off the box and cut her right thigh.
While working inside a Prill silo operating a pneumatic 30 lb air breaker, the Chicago fitting that attaches to the swivel came unthreaded. The swivel stopped working properly and caused the fitting to unthread. The air hose swung around inside the silo and struck the EE in the nose. This injury was a direct result of equipment failure.
While employee was making trim bundle he put sheet on top of trim bundle and started walking over to get another sheet to continue making trim bundle, walking past the starter sheet his left arm came in contact with the corner of starter sheet causing a laceration between shirt sleeve and protective arm guard.
When air arc a piece of metal off in a haul truck located in the mine shop, truck bay, sparks came up under the welding hood and safety glasses. Thus causing a small superficial abrasion on the 3 o'clock of left eye's cornea. No FB on cornea or conjuctiva.
Employee got stung by wasp while getting on fork lift.
