Employee states after shoveling at the Mill, he felt pain in his lower back.

Employee was bent over picking up an impact and twisting at the same time when he felt pain in his lower back.

While lifting PC equipment from dolly, employee felt pain in the lower back area resulting in a strain.

Employee was removing a bolster from rear of haul truck bed using a pry bar and a forklift. When bolster freed itself from the bed, employee slipped and twisted his back.

Over-exertion in lifting objects

While carrying a brake drum, employee felt pain in his right bicep. After medical review, nothing was discovered. Employee did not miss any time from injury. In October, employee's shoulder was operated on due to injury. Employee is now missing time from work.

Over-exertion in lifting objects

While lifting a chock block, employee felt some pain in his lower back.

Over-exertion in pulling or pushing objects

Employee was trying to move a chock block, chock block was stuck in mud causing him to pull harder, resulting in pain to his back.

Struck against a moving object

Employee started downhill in forklift leading from Mill to shop. He turned forklift around to traverse the decline in reverse for better visibility. Forklift began to slide due to wet road conditions. Forklift would not stop so employee turned forklift into berm where it stopped and overturned.

Fall to the walkway or working surface

Employee reports while walking he slipped in mud and fell causing pain to ankle. *This was not determined to be reportable until April 2005, due to a personal injury previously and other factors.

Struck by falling object

WHILE STRIPPING FORMS ON A NEW SUMP UNDER CONSTRUCTION, STRIPPING FORMS SLIPPED & FELL ON HIS ELBOW. HAIRLINE FRACTURE TO HIS ELBOW.

Employee stepped out of company van on loose gravel causing her to twist her left ankle.

Technician using wrench to loosen nut on acetylene gas cylinder. The wrench suddenly moved pinching the fifth finger on his left hand on the ring of the safety cap.

EE FELL BACKWARD WHEN HE STEPPED IN AN UNCOVERED NOZZLE. EE CAUGHT HIMSELF AND HURT HIS LEFT ARM. EE CONTINUED TO WORK FULL SHIFTS. ON 4/4/05 EE COMPLAINED THAT HIS ARM WAS STILL PAINFUL, SO HE WAS TAKEN TO COMMUNITY HEALTH CENTER. X-RAYS WERE TAKEN AND IT WAS DETERMINED THAT EE'S LEFT ARM WAS BROKEN.

After removing a broken bolt from the right final on a haul truck, employee went to wash his hands when he felt some irritation in his left eye.

EE WAS HOLDING PLYWOOD FOR JIG SAW OPERATOR. PLYWOOD SLIPPED AND EE'S HAND WAS PUNCTURED BY JIG SAW.

Another employee was swinging a hammer and the hammer glanced off a sheave and struck employee on right thumb resulting in a fracture.

Employee was tightening a cap bolt on automill high pressure lube filter when the torque wrench slipped causing him to loose his balance. While attempting to catch himself he grabbed an iron bracket and cut the palm of his right hand.
