Foreman went back to scaffolding and tied off to back rail, tried to pin and move beam over when structure clip slipped off of chime and employee fell with the beam. Employee was tied off to the structure.
Metal plate fell onto employees foot. Incident did not become reportable until 2/16/2026 when employee sought metal attention and was diagnosed with fractures.
Employee drilled a test hole and was removing drill steel from the top when a piece became lodged. As employee was trying to retrieve the stuck drill steel, it fell out of the hole and struck them on the left pinky, causing a laceration.
EE was inspecting engine bay on loader. Shocks on engine bay door loader failed. Overhead door fell and hit EE on top of the head.
The employee was walking up the Mill stairwell for lunch break and looked up. When the employee looked up, a piece of dirt fell into the employee's right eye, causing irritation. The employee was treated with a prescription eye drop at the on-site medical clinic.
Employees were removing a deflection plate from a lowered stacker. After communicating the plate was ready to fall, the fall zone was cleared. The plate was dropped and bounced into an employee striking a foot. Employee was transported to occupational health, x-rays were taken and found nothing broken. Health provider suggested over the counter meds and ice.
While service tech was performing the pre-shift maintenance the crusher. Exc opr was standing near the crusher 4-5 ft away in the area previously cleaned by the excavator, near rock pile. While EE was waiting for others to finish cleaning, A large rock slid off and struck the employee's right foot and caused two fracture and laceration.
EE was preparing to weld inside Screener 724 during light rain, with a tarp placed over the screener to keep the area dry. Before work began, a chunk of ice fell from the top of the birdhouse, slid down, and struck EE while EE was inside the screener, causing minor injuries to EE's chest wall, right upper arm and lower back.
EE was on the 5th floor of the mill washing when a rock (4" + size) came down from the 6th floor striking EE on the Left Elbow
Employee was in the Fabrication shop getting metal supplies for pipe rack when an 8 foot square tubing slipped off rack and struck right foot. EE was taken to medical facility day of injury. EE received second opinion on 11/30/2025 from medical professional where X-ray resulted in fractured #5 Metatarsal. EE is placed on light-duty.
Injured was removing Shim Pack from Ventilation Fan, the Shim Pack unexpectedly shifted striking the injured on the left wrist.
Employee was helping unload short sticks of rail at shanty when one rail rolled and pinched finger in between another short stick. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

While drilling hole for cable bolt in St Leo #3 126-128 block the steel got stuck in top, miscounted how many steels were in roof (removed 2 of 3 steels) and last steel fell out striking back of left hand causing open wound that required 11 sutures to close. No problems with equipment or compliance issues, training or PPE.
EE was conducting maintenance on the engine of an Epiroc haul truck when the strut that held the engine cover open broke, causing the engine cover (weighing approximately 100 to 200 pounds) to fall and impact the EEs hand between the engine cover and the chassis, resulting in the EE breaking their left hand.

Employee sustained a nondisplaced fracture of right thumb (Distal Phalanx) when employee was ascending the stairs to access the belt 2260CON001 and the safety bar came down and smashed the employees right thumb.
Liners fell unexpectedly, striking worker's leg. Causing bruising and soreness.

While inspecting a port on the tower the employee got hot dust in their face causing an eye abrasion and 2nd degree burns to face.
Went to change battery on a scoop when the battery slid off of the cribs and hit IE in the knee
While getting in coal hauler employee had right hand on fender of hauler, when a rock struck employees hand requiring 1 stitch. Employee returned to work the next day.
Employee was hosing the ground under the catwalk and a 3/4" rock fell from a pile and struck them on their shoulder.
Per employee: They were cleaning off Catwalk on Screen 1 like they were told to do. A big rock fell from the top of the machine and hit them in the back.
Employee cut a piece of plastic pipe that was hanging vertically and was secured above by coupling. When they cut they moved to adjust their fall protection when the pipe came out of the coupling and fell onto their hand, amputating two of their fingers on their left hand. Doctors were unable to reattach.
The inspection cover on the VSI was open, the employee bumped the plate causing the cover to shut. The plate struck the employee, resulting in a broken arm. Two conditions contributed to the accident (1) the inspection cover lacked an OEM locking mechanism to hold it safely open (2) the employee was new to the position and had not developed adequate safety awareness.
While waiting for the shift to begin the employee sat in a chair next to a cork board mounted on a wall. The cork board fell and struck the employee on the left shoulder.
Employee had drilled a hole to install a roof bolt when the drill steel became stuck. As they turned to get another piece of steel to retrieve the stuck steel, the stuck steel fell from the hole, catching their thumb between the steel and the drill pot.
While attempting to pry against a stuck rotary mill, an employee propped a large pry bar in the inspection port, while positioning to look into the hole, the bar slipped out and fell onto right thumb causing a contusion and a fracture.
Employee was greasing head gate sprocket when rib roll pushed EE into side of headgate causing EE to cut EE's hand. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.
Employee was on the lower platform of the control house, assisting in freeing a rock obstruction from the mouth of the jaw crusher. Employee had opened the top of the crusher cage so that the operator could manually free the obstruction. After obstructions were freed, the rock entered the jaw and the vibration caused the guard to fall and impact employee's left hand.
Employee hung a ventilation skin on the back of the section fan, and as they turned to walk away, a 10'L x 4'H piece of the inby coal rib fell striking them on right side knocking them to the mine floor. They sustained a cut to the forehead, and contusions and abrasions to the right side of torso and lower body.
Employee was looking at 988 wheel loader for customer. Machine had electrical problems, Hood on machine was raised, employee was looking at wiring under the hood when hood fell, striking employee on the head, knocking their hard hat off, and striking their ear causing laceration on the ear.
During the heavy rain down fall near the end of the shift on 2nd shift, a rock from the rock pile came sliding down the pile and struck the right-side window of loader, breaking out the side window and striking operator on left knee area causing a laceration to knee.

While setting a dewatering pump, the slope on the east side started to slough off into Rim Ditch. All were advised to extricate from this area. They egressed to the east slope and when it sloughed off to the bottom, they fell approximately 4 feet landing feet first on a rock. They hurt their right ankle and left knee.
Employee was changing a starter on a Rock truck; they had the starter placed on a step ladder. The starter rolled off and landed on their left foot behind the steal toe. Employee went to the doctor had a bad bruise told to rest.
EE had just finished a repair to the surge. When coming out of the chute, some small rock slid off the shelf and slid across EE's head. This left EE with a cut.

IE was building a brattice when it fell. When it fell it knocked them into a pallet of block that was on the group causing an arm injury.
Injured employee was hosting the chute below the scalping screen on the rotary breaker floor when a piece of rock fell and struck injured employee's hard hat causing a laceration requiring sutures to close. No time was missed as a result of the incident.
EE was organizing lower tire yard. While moving wheels to scrap, EE had stacked Wheel #@1 on top of wheel #2, when wheel#3 that was standing next to the stacked wheels fell into wheel#2 while EE's left hand was in the line of fire, resulting in a broken left ring finger above the first joint.
EMPLOYEE WAS OPERATING A HAUL TRUCK WHEN THEY FELT SOMETHING WAS OFF WITH THE TRUCK. THEY PULLED OFF THE MAIN ROAD AND CHECKED THE SWAY BARS AND WAS GETTING BACK IN THE TRUCK WHEN A PIECE OF LOOSE ROCK FELL AND GLANCED OFF HARD HAT HITTING RIGHT SHOULDER BLADE.
Customer Truck Driver became engulfed in Stone (DGA) when unloading excess material after being weighed out. Driver at the rear of EE's vehicle when slope of stone pile gave out trapping EE under the pile. EE was removed by Quarry personnel with assistance from Emergency Response Units. EE was evaluated and released from Hospital the same day.
On 1/25/25 at approx. 8:30am a drill operator was injured when a hook from the drill winch cable fell from the top of the drill mast. The hook struck them over the left eye causing a laceration over their left eye. EE was taken to the ER, receiving multiple stitches. The operator had a follow-up appointment on 1/27/25 was released back to work 1/28/25 without Restrictions.
