Two employees had set a beater return roller on a lower frame part of the stacker frame of the conveyor; the roller slipped out from the frame and came into contact with injured employee's index finger.

EE got blisters from a hot sheave. The bearing had gone out on a screen which heated up the sheave. The contact with the sheave caused small blisters on EE's hand.
EE was dismounting the excavator for the end of shift. EE had three points of contact while dismounting and slipped off the foot peg on the middle of the track, which is 18 inches off of the ground.

Employee was operating a front-end loader and jarred left arm in rough terrain. Left arm muscle strain. Did not become reportable until after Clinic follow-up on May 10, 2019.

Installing counterweight pulley on field conveyor. Tele-handler and come-along holding pulley on either side of conveyor. Come-along cable snapped. Pulley swung back and struck middle finger of right hand.

Using a 70 lb. jackhammer to release a wear plate in a Cedar Rapids MVP 280 portable crushing plant, cone. the jackhammer slipped causing the right hand little finger nail to bend back away from the finger. Stitches were needed to reattach grown plate of finger nail.
