Mostrando 50 de 862Employee was changing shaker springs on Shaker C in Plant. While installing the new spring, the shaker deck dropped and pinched left middle finger resulting a laceration that required 13 stitches to close wound.
While assisting a contractor that was repairing a scraper at A03 in the A Frame, a material handler miner got hit by a piece of pipe previously removed fell down the working platform while EE was standing in the underneath catwalk and resting EE's right hand on the fix ladder cage. This struck caused EE an open wound fracture to EE's right pinky.
Contractor was assisting repairs of coal mill rollers during outage. When shaft was being lowered onto sawhorse the injured who was assisting the moving of the shaft, pinched their right handed middle finger between the shaft and the sawhorse causing a laceration in their finger. Contractor Supervisor took the injured to get medical attention. Receiving 10 sutures.
Around 2:30 pm, a mechanic while working in the FM1 separator at the time of positioning a come-a-long above their head, dust got loose and fell into their left eye. They flushed their eye and were taken to Urgent Care where medical evaluated and diagnosed with a corneal abrasion of the left eye, without a foreign body.
EE was lifting a full oxygen bottle into a UTV and during this task the bottle slipped pinching their right ring finger in between the bottle and tailgate causing a laceration and fracture of the finger.
In occurrence while EE was assisting an EE, this EE stepped backwards onto EE's left foot causing a closed displacement fracture of the third metatarsal bone.
A laborer working in the Finishing Mills was cleaning a gypsum spillage. The employee was filling and carrying two five-gallon buckets; the employee stated their back was hurting and had trouble standing and walking with a sharp pain. They were treated for a back spasms.
Employee was adjusting the drag cable on the drum when EE's finger became pinched between the wire rope and the drum. This resulted in a fracture with stitches to the left index finger.
On November 20, 2025, at about 3:00 p.m., the Maintenance Clerk was repositioning a 24x20-inch picture frame when it broke. Falling glass struck the employee's left kneecap, causing a one-inch cut. The incident was reported immediately. The employee received four stitches; no restrictions were issued.

They took a guard off saw a rock was lodged in a rotary gate, and decided to save time and effort used a finger to dislodge the rock instead of picking up the bar. The rock had moved allowing the rotary gate to rotate, and they couldn't move their finger fast enough.
Subjective c/o generalized back pain after transferring 1/2-full 5-gal buckets ~3-4' from 3'H ledge to coworker; no lifting, stooping, or twisting required. Mandatory weekend shift; EE wanted to leave early, complaint is suspect. EE refused post-incident drug screen, admitted THC/marijuana use. Terminated for cause per zero-tolerance drug/alcohol policy.
A Dozer Operator with 24 years of experience, was dismounting the dozer before EE's lunch break. Employee stepped from the track down to the first step, left shoulder popped as EE descended
Miner was working in Kiln 1 ballroom doing routine housekeeping, EE went to lift by self a 3x3 door (weight 50 pounds) used to return cleanup material to the silo. About an hour later EE felt a sharp pain in EE's groin and a dull pain in EE's lower abdomen. EE continued to work and finally reported the incident at 4:50pm
EE was lifting vactor hose and felt back pain.
EE was in the process of ascending on to a closed cab skid steer. As they attempted to open the door of the cab of the skid steer, the EE lost footing, pivoted to the side as they held on and then fell on to the ground resulting in a laceration to the upper part of the EE's head.
EE was replacing drive belts and caught left ring finger, resulting in the loss of nail.

Employee was attempting to position a motor base when it moved unexpectedly and pinched EE's finger between the base and a pivot table.

EE was attempting to clear a dust collector vent line and in the process of doing so they got a fragment of material in eye. Although they were wearing safety glasses the material got into left eye. The material was removed at the clinic and the EE was given an eye patch and placed on restricted duty for 2 days.
EE was attempting to close passenger elevator door, in the process of doing so EE smashed middle finger. Injury was treated as a first aid. On 03/10/25 EE requested medical evaluation which resulted in EE's duties being restricted. EE was also given splint to immobilize finger.
EE flip a manual gate to chute EE opened the metal door and secured the safety chain; EE bumped the metal door causing the chain that secures the door to come loose, the door fell back down and smashed the employee's finger The EE was wearing safety gloves when incident occurred Employee's finger index nail was smashed and employee sustained a fraction finger
Employee was troubleshooting issues with material flow at a cement finishing product mill. During this process the employee opened an inspection door to an air slide and when EE opened the door a flush of material came out and got in EE's eye.
Employee was standing next to the hood doors on K2 Burner Floor when a 6" metal braided hose that was being suspended broke loose causing it to swing striking the employee in the face.
While working on the Preheat Tower, some material/dust particles fell from above and got in the employee's left eye.

EE felt a sharp pain near EE's sciatic nerve when EE pushed a plate.
EE was working as a tool handler when EE felt dust in eyes. EE visited a clinic on the same day to receive treatment. The doctor confirmed that it was dust that had caused the irritation in the eye. The doctor washed the eye and prescribed some eye drops. As per doctor's order EE resumed work after the treatment on the same day.
Employee was removing a cement bag that was lodged on a conveyor belt and when the bag came loose it caused cement to blow onto employee's face falling in EE's eyes. Causing redness and inflammation to EE's right eye and eyelid.
Employee alleges that the employee was placing an iron tripod to place a cell phone to record and the legs of the tripod and the base were sapped, impacting the left hand, causing fractured nails and bleeding in the index and middle fingers.
Employee stated that was working with a driller to put the hose for the sump pump into the lake, when EE felt a sprain/strain type feeling in their back. When positioning the hose, body position can become awkward requiring twisting.
Employee was handling a bag of ice and scraped their left pinky at the cuticle area with a wire used to close the 40lbs bag. They notified on 02/09/2024 that they felt pain and has redness and swelling at the area. Antibiotics and vaccine were prescribed.
Employee was checking the gearbox to pinion alignment on a ball mill. EE was turning the coupling when the stored energy in the drive system caused the coupling to counter-rotate resulting in EE's hand slipping. The right pinky finger was pinched between the 2 coupling hubs causing a laceration and fractured and displaced bone.
The employee pinched two fingers in a Coal mill rotor while checking for tight spots. This resulted in lacerations on two fingers requiring stitches. The employee was released to work with modifications of light gripping.

Worker was removing a rock from an airlock when the rock slipped and smashed their left middle finger between the rock and the housing. This caused a fracture and laceration to the employee's finger. Employee was placed on modified duty.
Team lift of a purlin being performed when wind gust caused one employee to lose control of the purlin, which rolled onto right wrist lacerating it. Leather gloves & long sleeves were being worn; proper team lift techniques were followed. Bleeding was slowed and employee transported to ER for treatment - 10 stitches and a tetanus shot. Released with no work restrictions.
Maintenance Mechanic was replacing an elevator bucket, when handling the bucket, the bucket shifted causing a laceration on EE's left ring finger.
