Employee was on his knees while bolting the #2 entry. He was positioned at his previous drilling location. As the bolter trammed forward, the side of the bolter hit his rescuer that was located on his left hip causing his left knee to twist into the mine floor.

EE was holding a wash hose washing off a scoop. The pressure pump kicked on and the nozzle was pulled from his hand causing a laceration.

Injured member was operating the roof bolter when he placed his left hand on the canopy and smashed his middle finger between the canopy and roof resulting in a laceration requiring 5 sutures and a distal fracture of the left middle finger. He was released to return to work.

Member was bolting in 2 left putting up the 2nd bolt he was putting glue and bolt into hole when a small rock hit the canopy and then causing a small cut to his face. He worked his next shift and then missed 2 days because he sprained his neck when he seen small rock falling

Part 50 Audit. Employee was using a cutting torch when the oxygen hose failed causing the employee to receive 2nd degree burns to his abdomen and upper right arm requiring cleaning and medical treatment of the wounds.

Employee had bent an eight foot resin bolt due to seam height. The roof bolt would not seat in drill chuck. He hit rotation and the roof bolt struck him in the right wrist resulting in a fracture.

Employee was running a Caterpillar D10R Dozer. As he was backing up, he struck a 3' high pile of coal which gave him a jolt. Employee claims he began to see a black spot in his right eye.

Drilling a bolt hole and smashed right middle finger between drill pot and drill steel.

EE was setting feeder on to tail piece on the 002 section. While cleaning out the gob underneath the cats to put the cribs in place the EE slipped and fell forward. EE put his left hand on the raised jack to catch himself. At this time another EE let the jack down and pinched the EE's left ring finger between the jack and the frame.

Employee was reaching upwards installing a rib bracket when he felt a sharp pain in his lower back. Employee finished his regular shift. He went to the doctor the next day.

EE was bolting in the #3 heading. As he was putting up pressure to install a bolt, a piece of metal that was welded to the machine broke off and struck him on the right side of his head. No visible injury.

EE was drilling a hole for a roof bolt when the drill steel stopped up. There was pressure on the drill head which caused the drill steel to bow outwards. When the EE released the pressure the drill steel kicked back striking his left hand. This resulted in a hairline fracture.

Continuous Miner was turning Left Break in #5 Entry. While loading shuttle car a piece of rock came off of the boom of the miner and struck the employee on the right side of his face. A laceration on the right cheek bone required 4 stitches.

He was putting up a roof bolt the end of the bolt broke. He drilled another hole beside the bolt that had broke the drill steel drilled into the broke bolt, when he let the head down the drill steel hit him on the left side of his neck.

Member was operating the continuous miner when a rock came out of the boom and struck him on the lip. Member immediately ceased work and transported himself to the hospital where he received 4 stitches.

Employee was plastering stoppings using a spray plaster machine. He turned machine on, the relief valve was open and the plaster got into his eyes. His first day away 3/7/2011.

The employee was pushing a rock out of the way with the stab jack on the roof bolter. The rock either broke or the stab jack slipped over top of the rock, causing the drill head to move forward quickly catching his left thumb between the drill steel and the canopy post.

Member was in the deck operating the bolter. He went to reposition himself and stuck his hand on top of the canopy to pull himself up and hit the tram. The bolter moved, smashing his right hand into the top. Member immediately ceased to work and was taken by ambulance to CAMC General where he was told his right hand/middle finger had to be amputated.

Member placed his right foot on the addcar to unhook the cathead. When the miner operator saw the cable get tight he trammed forward causing the member to get his right foot wedged between the jack and the car. This caused three (3) hairline fractures in his right foot and was hospitalized overnight for observation.

Electrician was at the section power center drilling a larger hole in a ring terminal. He was holding the terminal with his left hand and the drill in the right. The drill bit caught his glove and lacerated his left thumb. He continued working and later sought medical treatment. The cut required 7 stitches. The safety department did not receive the report until 2/15/11.

The individual was backing the roof bolter out of #4 Left and his right hand was caught between the operator deck and the coal rib. He did not start missing work until 2-11-11.

Injured member was cutting an overcast on the #3 section in the #3 entry when a piece of rock fell or was thrown back and struck member in the left shoulder (Clavicle)causing a laceration. He was transported to the ER via EMS. He received 24 sutures and was released to return to work on his next shift.

The individual was roof bolting and had finished drilling the hole when the drill steel fell out of the hole hitting his left hand causing a fracture to the ring finger.

Member walked up and asked the miner man how much more he lacked so he could come across and clean that side of the section and the cut where the miner was fell in on the miner and a piece of rock slid down and hit member on the ankle. He was taken by ambulance to Raleigh General where he met Analabs for a drug screen and was scheduled for surgery on 1/29/2011.

Employee was installing a rib bracket with roof bolter when the coal rib popped sending particles into his right eye. Employee was wearing safety glasses. Employee continued working the rest of his shift. He stopped by and had his eye washed out and checked at Whitesburg A.R.C. Hospital on his way home.

The individual was welding on a personnel carrier and received a flash burn to his eyes.

EE was bolting in #9 entry; getting ready to set a bolt. While spinning the bolt, it came back and struck his Right wrist. This report is to abate citation 8154313.

The individual was roof bolting and misplaced a 5' piece of drill steel. The drill steel lodged in a fan bracket and when the individual swung the drill head out the drill steel flew up and struck the employee in the mouth. No medical treatment has been sought by the individual.

Member was swinging drill boom in when he started to fall, he was trying to catch himself by grabbing the top of the canopy with his left hand. Member continued to fall into the control levers and raised canopy into roof causing lacerations to his middle finger, which lead to doctors amputating the tip of his middle finger.

During roof bolting operations, the individual states he had his hand on canopy handle and when he swung the drill head out, the canopy came in contact with the roof mashing his left ring and pinky fingers.

Pinsteel steel stopped up, tried to get steel down, had left hand on drill pot, steel fell out striking him on the back of hand.

Injured employee was placing a crib block under the continuous miner cats when the operator raised the miner head and smashed the injured's left hand between the cats and block. Diagnosis: Fractured left little finger and 4 sutures on the posterior side of the left hand.

While using a jackhammer to break up a concrete floor, the jackhammer fell and when he tried to hold onto it, it jerked his left arm and shoulder. Caused a strain to his left shoulder.

Was helping put augers into beams and twisted the wrong way causing ee lower back to knot up. Didnt really feel anything until Friday morning around 10:00 A.M.

The individual was operating the continuous miner and positioned outby the boom of the miner wearing safety glasses, when an object was thrown by the cutter head, striking himi in the head above the left eye. This is a medical only report.

Employee was operating drill and looking out to find flag marker on the hole. As he closed the door, he got his right hand caught in the door causing an avulsion to his right middle and ring fingers that required a total of 10 sutures.

While operating the continuous miner, a rock was thrown by the cutter striking the individual in the head above the right eye causing a contusion.

The EE got down on his knees while operating the CM. When the EE started to stand up, he stated that he had pain on the backside of his knee. The EE stated that he did not know what happened that his knee just started hurting. The EE cont. working until 11/09/10. Note: The employee did not start workers compensation until 11/09/2010.

Member was welding and looked at the arc and accidentally burned his eyes. He was unaware of this until he went home and tried to sleep. His eyes began to swell after a few hours.

EE was removing brake pack from Komatsu loader using a 1" drive impact wrench with a made up socket keeper (made from welding rod) while operating wrench the keeper caught his glove and cut his left hand index finger.

Employee was cutting a bolt out of the section head drive motor. He laid the cutting torch across his legs (he was sitting cross-legged). He reached up with his left hand and torch fell off his leg and landed on his hand. He was taken to ER and treated for minor burns and was released to return to work on 10/11/10.

Employee was welding and leaned to get another welding rod. When he did, his neck and arm contact hot metal causing a burn to the right side of his neck. He continued to work until 9/28/10 at which time he sought medical treatment for an infection to the burn.

While pulling beams out of hole, a pin that connects beams together caught on blade, top of pin broke off, striking Member in chest and chin causing a laceration. Member was transported to area hospital where he recieved stitches to his chin.

Member was spot bolting on 3 A trac operating a single head bolter, he swung the drill pot over mashing his middle finger on his left hand. This required sutures to repair the wound. He returned back to work on his next scheduled shift

Employee was replacing the center section of a 35C Fairchild Scoop. While grinding on the center section the grinder kicked back hitting his upper lip causing a laceration. He completed his shift and that evening went to Raleigh General Hospital for further treatment and he received four stitches.

EE was spot bolting in #3 entry. He had his hand on the joystick & his right hand on the bolt. He said he was straightening the bolt to go in the chuck. His hand was between the plate & roof bolt when he engaged the joystick. His metacarpal glove caught the plate & twisted his arm with it. He was taken to CAMC where surgery was performed on 8/18/10. Bolt was a 4' resin.

The individual was tramming the continuous miner over a piece of coal. The coal burst and a piece flew up and hit the individual in the mouth breaking a tooth. This is a medical only with no lost or restricted work days.

Injured was installing roof bolts when the drill pot became lodged and a 4 foot finisher (steel) flew out of the tray and struck injured across the nose. Injured drove to the ER and was diagnosed with a nose fracture.

The individual was spinning roof bolt and a piece of foreign matter struck him in the right eye. The individual was wearing safety glasses. The individual had to have the foreign matter removed by a physician. This is a medical treatment with no lost or restricted workdays.

Employee was in the process of bolting using a two piece steel. After he drilled his starter hole he started his two piece steel and had his index and middle fingers caught causing a laceration.
