Customer truck driver found unresponsive in the cab of the customer truck driver's truck.

A third-party trucking contract driver was found unresponsive near the mine exit. Trained CPR providers from the mine responded and performed CPR until EMS arrived on scene. The driver was pronounced deceased by EMS staff.

A non-miner customer truck driver was knocked unconscious when a gust of wind blew their trailer doors open and slammed them, headfirst, onto the ground. The driver received medical attention for a concussion.

Injured had just dismounted their haul truck with both feet on the ground. When the employee turned to walk away the left knee buckled. Diagnosed with a left knee strain.

A customer truck driver was found unresponsive in the cab of the driver's truck on the US Silica customer pickup dock. The onsite emergency responder and 911 were notified. CPR was conducted until the ambulance arrived. The driver was then transported to a local hospital and was pronounced deceased due to a health related incident.

A deceased body was discovered in the South Ottawa Pit. The person is not a mine employee. The Sheriff and MSHA were notified immediately upon discovery and the investigation is ongoing. The birthdate of the decedent is unknown.

Employee knelt down to look at parts on the floor and felt a twinge in their lower back. Placed on restrictions.

Death due to non-mining related natural causes.

EE was operating a front end loader in the clay shed. EE opened the door and thought it latched open. EE reached out to get some food and the wind blew the door shut striking EE's left hand between thumb and pointer figure. EE required 8 stitches.

Employee was originally thought to have suffered from heat stress; however later it was found to have been caused by a personal medical condition.

Employee stood up after opening a rail car gate and felt a "pop" in knee. Employee initially refused medical treatment. Employee requested medical treatment on May 9th at which time surgery was recommended. On June 4th 2018 employee went off work for surgery.

A shift in hearing was reported during an annual audiometric examination. There have been no significant noise exposures according to our dosimetry records.

Employee was delivering a small sample bottle of sand to the lab and was descending the stairs at the end of the D5 conveyor when EE heard and felt a pop in EE's right knee.

A commercial truck driver for SDA trucking Had an apparent heart attack that resulted in his death on 11/10/2014. This incident has been investigated on MSHA Event # 6547649.

Employee reported that after he slightly bent over he came up and felt a pain in his lower back. He reported he did not lift anything and had pain in the past.

Employee injured knee while walking as he served as a Miner's Rep on a regular MSHA inspection. Injury did not require modified duty until November 8, 2014.

Employee twisted to the left and experienced pain in his lower right back. Employee was put on restricted duty from 8/26/14 till 9/2/14 and returned to full duty on 9/3/14.

Squatting down to move into work position and felt knee pop. Treated using first aid until 9-26-14 when it became reportable due to medical treatment.

A Service Worker suffered a fatal heart attack at a residential property owned by the company and adjacent to the active mine site. The Service Worker was removing an inactive LP gas tank when he fell ill. EMS was called by his coworker, and the victim was rushed to a local hospital. The fatality was due to natural causes and was not related to any mining activities.

Employee was walking and went to make a turn and his knee gave out. The employee reported that no single event occurred to cause the incident, according to the employee other than walking. There is no work relation to the injury other than being on mine property.

Employee became ill and was subsequently transported to Limestone Medical Center

Employees was opening a discharge slide on a rail car when wind blew sand into his left eye, causing an abrasion to the cornea.

Customer's over the road truck driver was found unconscious near the truck scale.

Employee was struck in the eye by debris while working either on a door in the shop or while working on packing for a pump, both jobs were on separate days, the employee is not sure when it happened exactly. Employee reported incident to his supervisor Sunday February 28th and stated that he wasn't sure if it happened Friday February 26th or Saturday February 27th.

Accident is under investigation. Employer has not received any medical results.

As employee broke for lunch he started to feel pain slowly come on in his neck and left arm. The employee has a serious personal illness and has not been to work since 7/18/2007. The company received paperwork from the employee's doctor that said he is off of work for this injury not his personal illness on 8/2/2007.

Employee popped his right knee out of place doing maint. work on a loader. He indicated that the damage was from a pre-existing condition caused by a skiing accident and that the "popping" of his knee happened often. Injury was ruled a first aid at the time-no lost work days. Employee went for surgery on April 26, 2007 causing the first aid to become a LWC on April 27, 2007.

EE was found to have died from heart failure, natural causes determined by coroner.

Employee was using a step ladder to change light bulbs in main office. He over extended his normal reach, rather than move the ladder closer to his work. He felt pain in his back at the end of his shift and went to see a Doctor.

HIGH WINDS ON TRACK CAUSING AIRBORNE MATERIAL (SAND). EE'S EYE VERY SCRATCHY. EE SAW DOCTOR AND RECEIVED DROPS.

EE WAS CHANGING BAGS IN DUST COLLECTOR. HE STOOD UP & PLACED LEFT HAND ON RAIL. WIND BLEW THE COVER CLOSED, TRAPPING HIS HAND. THE RESULTING INJURY WAS A FRACTURED 3RD & 4TH FINGER OF LEFT HAN D.

EMPLOYEE WAS WALKING UNDER A CONVEYER, WIND BLEW DUST INTO FACE. FOREIGN BODY WENT INTO HIS EYE.

EMPLOYEE WAS TALKING WITH ANOTHER EMPLOYEE IN THE CONTROL ROOM OF THE MILL, WHEN HIS HEAD WENT BACK AND HIS EYES ROLLED INTO HIS HEAD. THE OTHEREMPLOYEE TRIED TO SHAKE HIM AND GOT NO RESPONSE FROM HIM. THE EMPLOYEE HAD NO PULSE AND WAS NOT BREATHING. THE EMPLOYEE HAD SOMEONE CALL 911 AND BEGIN CPR.

PLANT OPERATOR HAD SHUT DOWN THE PLANT AT THE END OF THE SHIFT & WHILE PERFORMING CLEAN-UP ACTIVITIES, SUFFERED A HEART ATTACK

WHILE WALKING THROUGH TO THE CONTROL ROOM, WORKER GOT DIRT IN HIS EYE FROM THE WIND.

EE WAS WORKING ON A D9N BULLDOZER. HE HAD HIS SAFETY GLASSES ON BUT THE WINDY CONDITIONS CAUSED SOME DIRT TO BLOW INTO HIS LEFT EYE.

EMPLOYEE WAS WORKING ON DUST PIPES. A FOREIGN OBJECT FLEW INTO HIS LEFT EYE.

WHILE HELPING WELDER, DIRT BLEW INTO LEFT EYE. EE WAS WEARING SAFETY GLASSES.

WHILE WALKING OUTSIDE NEAR MILL #2, WIND BLEW DIRT INTO HIS EYE.

I WAS DRIVING MY FORKLIFT ON FRONT DOCK GOING TO POWDER WAREHOUSE, THE WIND WAS BLOWING AND IT BLEW SOME DIATOMACEOUS EARTH INTO MY LEFT EYE.

EE WAS CHANGING AIR HOSE ON DRILLING RIG. CONDITIONS WERE VERY WINDY, AND A DUST PARTICLE BLEW IN HIS LEFT EYE.

EMPLOYEE STATES THAT AFTER HIS SHIFT HIS EYE WAS IRRITATED. AFTER HE WOKE UP, IT WAS STILL HURTING AND DECIDED TO GO TO THE DOCTOR.

EE WAS WALKING THROUGH #1 MILL ROOM. THE WIND BLEW A FOREIGN OBJECT INTO HIS RIGHT EYE.

EMPLOYEE WAS UNPLUGGING BAGHOUSE CONE IN VERY HIGH WINDS AND GOT DEBRIS IN LEFT EYE.

WHILE CLEANING ORE OUT OF THE TRUCK DUMP (1-1 GRIZZLEY) THE WIND BLEW THE PLASTIC DOORSTRIPS INTO THE WORKER'S FACE. SHE FINISHED HER SHIFT AND WORKED 3/4 DAY THE FOLLOWING DAY. THEN WENT TO D OCTOR WHO SAID SHE COULDN'T WEAR A RESPIRATOR FOR A FEW DAYS.

EMPLOYEE WAS WALKING THROUGH THE SHOP AND FELT SOMETHING IN HIS EYE.

WHILE WALKING DOWN STAIRS DUST BLEW INTO WORKRERS EYES. RX SALVE.

EE AND ANOTHER EE WERE INVOLVED IN A DISAGREEMENT CONCERNING A HAUL TRUCK BLOCKING THE ROAD. INJ EE ALLEGES THAT OTHER EE SLAPPED HIM ON THE SIDE OF HIS FACE. HIS NECK WAS SORE AND STIFF (SEE ATTACHED)

EE BEGAN LOSING TIME FOR 1994 INJURY.
