EE was exiting EE's haul truck at the end of EE's shift. When EE stepped down to the ground EE stepped on EE's ankle wrong and fell down. When falling EE's right elbow hit the ground. EE was taken to OMP on 8/17/26 for further evaluation and placed on restricted duty.

Employee slipped and fell against a handrail bruising EE's right ribs

There were no conditions that contributed to the injury

While walking the employee tripped on the catwalk grating section that connects the two different platforms and fell forward, landing a knee on the grating. The employee got up and noticed they had a laceration just above the kneecap.

Employees were removing jacks from under #2 kiln tire (jacks free of any weight). Employee was guiding jack off the base; it came off awkwardly and pushed EE off balance. EE placed EE's right arm out to catch myself and felt a pop in EE's shoulder.

Employee cleaning dust off ground with air hose and lance, as employee stepped over truck guide, they tripped. Causing the person to make contact with a SafeRack pedestal with right knee.

On June 19, 2026, an employee was descending a fixed ladder on a crusher when EE lost EE's balance and fell to ground.

When using a chain binder to chain pipe to floats, the employee lost their balance and fell off the pontoon, landing on the ground. When falling they knocked the cheater pipe off the float they were using to tighten the binder, causing it to fall and strike the employee, causing a laceration to the forehead.

a loader operator, was climbing down from the 980H loader, as they stepped down with left foot they felt a wobble. The next morning their left knee was tight and stiff. After receiving the MRI results, 7/10/26, it was determined operator has a slight meniscus tear. This is why it wasn't reported prior to now

Employee was installing a door at the backend of the kiln. They knelt down to retrieve a tool from a coworker and rolled their right ankle. Resulting in an ankle sprain.

Emp. was standing on the track hoe tracks filling with DEF. When EE finished, EE turned to the left and EE's left foot slipped off the tracks and EE fell landing on the fuel trailer that was parked beside the track hoe. EE suffered 4 fractured ribs and nicked EE's lung on left side. The area received a heavy rain the night before, the area, the tracks, and EE's boots were muddy.

Employee stepped off onto the platform on the 826 miner to descend using the access steps & stairway. The bottom steps were in the raised position and moved downward when weight was applied, stopping abruptly. The sudden movement caused the employee to experience pain in the lower back and umbilical area.

The operator stepped down off a minor platform and twisted an ankle.

EE tripped going into porta potty, and fell to the ground.

Employee was de-icing equipment and slipped on ice resulting in 2 fractured ribs.

Employee was being transported to the west pit by a supervisor when they stepped out of the truck and slipped on ice and landed on right hip and right elbow. After onsite evaluation it was determined to take them to the ER for imaging to ensure no further injuries. Employee was diagnosed with a strained shoulder, put on restricted duty and instructed to follow-up with an ortho.

Employee was tightening bolts on crane wench. Socket came off bolt and employee contacted ground. CT scan positive for LT pelvis fracture.

Employee slipped and fell on a patch of ice exiting a doorway on the silo tops near belt conveyor 12. Employee reported incident 1/5/26, sought medical 1/28/26 and was placed on restrictions on the follow up appointment on 2/3/26.

Stepping onto boat and step gave away and fell onto left side, rib area.

Employee was walking from the shop office back to the garage. There is a small depression where the concrete slabs meet, and the employee tripped and rolled their right ankle. Employee was immediately taken to ER and will be out of work due to restrictions.

Employee experiencing shoulder pain on 12/3/25 from a preexisting injury that occurred on 4/18/25. Employee slipped on ladder descending from 70-ton truck. Right hand slipped, quickly re-grabbed rail, felt pain in right shoulder.

EE slipped on a patch of ice falling onto EE's right hand that was holding a hammer. EE fractured the middle, ring, and pinky fingers of right hand.

While climbing into the D8 dozer, an employee's foot slipped. As a result, their weight shifted onto their left shoulder, causing pain.

Employee was using water hose to clean out under crusher when there was a change in water pressure causing employee to fall forward. Employee received 5 stitches to LT eyebrow. Discharged to full duty with no restrictions and no prescriptions.

Employee was dismounting a ladder. As they descended, they missed the 2nd step catching themselves with their right wrist.

Employee was walking in tunnel when EE slipped on EE's left foot and EE's right foot stayed put. EE felt a pop in EE's right knee and felt pressure.

While miner was working, miner tripped and fell on shoulder (left) and felt a pull Miner asked to go to Doctors on 9/4/2025 where EE was put on restricted duty and prescribed a sling.

On 8/12 employee was descending tower stair case and slipped on step and twisted lower back. On 8/19 it was brought to our attention as they wanted to seek treatment .

While working on 1-1/2 # 2 screen. Employee slipped inside chute while unbolting old skirt. Employee used right arm to brace self causing injury to upper right arm/shoulder area. Employee did hit floor. No subsequent injuries due to hitting floor.

Employee was getting out of their truck to use the restroom and missed the first flex step and fell to the ground landing on their butt and lower back. Accident currently under investigation.

Driller slipped off drill rig falling approx. 2'. Landed on feet and jammed right knee.

Employee was climbing out of plant 2 tanks on a ladder with 3 points of contact. 1 point was plunger rod. The rod is bolted on the bottom. Employee slipped off plunger rod causing a rapid fall, cutting EE's leg, took 7 stitches

EE had just exited the excavator and walking down the slope to the Trommel when material gave way causing EE's knee to buckle. Conservative treatment provided up until 6/26 when EE received MRI results showing a meniscus tear. Resulting in an upcoming surgery.

While descending from a roof bolter, EE planted EE's right foot at the edge of the cab when EE lost EE's footing and missed the top step, causing EE to fall approx. 5 ft. onto EE's right shoulder. On 6/3/25 EE was evaluated at Ohio Health Work Health where EE was diagnosed with a strain and given pushing/pulling/lifting restrictions of 10lbs with right arm.

Coming out of a trailer the employee tripped on the top step and fell to the ground on their right shoulder. The fall was 3 steps high and approximately 3 1/2 feet. This became a lost time incident when the employee received surgery on 7/2/2025 for a rotator cuff tear.

Warm weather conditions in daylight. Shoveling on catwalk to clear bridged material. Catwalk was free and clear of material where employee was working to clear bridged material in chute.

Operator was changing the bottom deck top screen in our KPI screen as EE was climbing out from under the rubber flap on the rear of the screen the worn screen tipped over which struck EE on the back left portion of EE's head which caused EE to strike EE's right side of EE's head against one of the protruding bolts on the rear of the screen

During the process of moving railcars an experienced employee needed to maneuver a rail switch to change the track. The shaft of the rail switch was not easy to maneuver, so the employee applied additional force by stepping on the switch. The switch went into place unexpectedly and the employee felt/heard a "pop" in left calf. The Employee was given a prescription medication.

Employee tripped on a stone and landed on back and got cuts to arm. Employee went to the ER and got looked at and got 10 stitches on the arm. Employee was released.

Employee was servicing equipment, EE started to descend the ladder. When EE got to the last rung, EE's left foot going to the ground and as EE was letting go of the rails with EE's hands and stepping down with EE's right foot...EE caught EE's boot solidly on the last step which caused EE to fall back. EE's body twisted as EE was falling, landing EE on EE's right hip and back.

Employee was performing maintenance activities inside the Barge Jaw Crusher. EE was standing on a piece of plywood that shifted, causing EE to lose footing and fall into the jaw area. EE was wearing fall protection and SRL (Self-retracting lanyard) that stopped EE from falling farther into the jaw crusher.

Employee was getting off a track screen and stepped on a frozen sand clod on the track and fell 24 inches to ground and twisted left ankle.

Employee was conducting pre shift inspection on sand plant, slipped on ice, fell and broke their left wrist.

EE was coming down the plant from cleaning the walkways. When EE got behind the old #1 screen EE slipped on ice. EE went to catch self and tore muscles in shoulder.

While walking to the long reach excavator, employee twisted knee while stepping through ruts in the frozen mud. EE feels some discomfort but refused medical attention at this time. Miner went to EE's doctors on 1/8/2025 and was prescribed physical therapy. No restrictions/ no time lost.

Employee was walking back to the main office in the main parking lot and stumbled causing them to fall, they landed on their right hand and fractured it "boxer's fracture".

Maintenance employee fell while pulling on a 36-inch pipe wrench at the secondary crusher. The wrench slipped and employee fell back on a hose reel and then forward resulting in pain to middle back and left shin. Employee was prescribed pain medication.

While clearing a rock jam in the crusher, the associate lost EE's footing, fell backward, and struck the side wall. EE then slid down to the entrance door of the crusher. After being assisted to EE's feet, EE walked approximately 40 feet and then rested on a walkway. EE expressed concerns about tingling in EE's feet and legs.
