An employee was tightening countersink allen bolts on a rock shelf for an impactor. TM was holding an allen wrench on the bolt head while another TM put an impact gun on the backside nut. Force of impact gun on the nut threw the allen wrench from TM's hand toward EE's mouth and fractured a tooth.

Miner was starting up the crusher, had just powered everything on in the MCC then went up into the control tower and heard a loud pop noise and saw smoke come out of MCC knew there was a fire and evacuated and called supervisor. Fire department and power company came out to shut off electrical and put out the fire, fire department is investigating results pending.

EE was stepping back after hooking up rigging and stepped into a low spot which caught EE's foot as EE fell. EE applied ice and rested it for 6 days with little improvement so got x rays on 7/27 which showed a broken bone in EE's ankle. EE is cleared for light duty work at this time.

Miner was shoveling light build up off of a catwalk. Felt something in neck that brought them to their knee. Requested medical, CT scan was done. No known injury, miner was released back to work. Pain continued went back to Dr on 07/20/2026 and got a doctor note requiring time off/ modified duty.

Employee setting concrete forms reported feeling ill after lunch. Supervisor suspected heat-related symptoms, placed employee in vehicle with cool air, provided water and electrolytes, and observed EE for the remainder of the shift. Symptoms returned that evening; employee went to the ER, was diagnosed with dehydration, and received treatment and observation.

Moving crane pad into position, individual strained their lower back.

Team member climbed onto a water truck to check the water level. As they were climbing back down, their left ring became caught on a step, causing a snag incident.

Pulling cable out of the water during repairs, lead to a strain in lumbar region.

EE was operating an aerial lift around the plant when they ran over some uneven ground the platform produced a sudden movement causing the EE to strike their left knee against the railing frame.

During a material handling operation involving a Hydraulic cylinder and employee was struck by the load resulting in injuries requiring offsite medical treatment.

Struck by/against fixed structure that gave way to a tensioned come-along. Employee was struck in lower back, causing bruising and pain in the area. The injury was at a tolerable level and could be managed with home care. No other persons/equipment effected

Miner was looking for an electrical issue on a ladder. EE missed a step on the way down and fell 3 rungs to the ground landing on EE's shoulder. EE was shoulder and dislocated it. EE was carrying a flashlight in right hand.

Miner was crouching down between support legs of conveyor to look under jaw and then stood up and made contact with crossbar that was part of the support legs. The contact led to a broken nose and cut lip. Stitches required.

Miner was at end of shift and stacking unused metal pieces after working on fabrication project. Miner was walking on metal sheet that was on the ground and fell, the fall resulted in a broken lower right leg. It was dark in this area, miner was using a headlamp for light. Medical services responded and took EE to the ER, miner required surgery.

Cutting 4 inch water hose with multi tool, tool slipped and cut hand through glove. Leaving roughly a 1 inch cut.

Outside hauler from Brix dropped off material at rap pile and left box up while pulling away, resulting in the box making contact with an overhead 240 powerline pulling it down. Power line fell on top of the hood of the Brix truck. Hauler stayed in the cab for more than 30 minutes until a qualified electrician from PGE removed the power and clear the scene. No injury.

Team member was tightening a set screw in a slightly bent position, stood up, and experienced back discomfort.

Tunnel feeder had concrete chunks get stuck on crossbeams and needed to be removed before any more material is fed into the feeder. Employee took a 12lbs sledgehammer and tried to break material, this did not work. After this task the Employee felt pain in their right hand. Employee did not report the pain until 03/27/2026 at 630am after medical treatment

On February 20, 2026, at approximate 7:30 am our miner suffered a fall due to not wearing fall protection injuring an ankle, ribs, back and head,

Employee was assisting with the replacement of a head-drum shaft bearing on the #1 screen conveyor on the Portable Plant. The chain that was used to support the head-drum and shaft assembly snapped, striking Tyler Whipple middle finger on left hand, fracturing the 3rd digit / tip of finger.

EE was removing a punch plate from the shaker tower when the plate moved unexpectedly pinching two middle fingers on right hand against the frame of the shaker.

While stockpiling with a front-end loader, miner felt miner pain in lower back. Towards end of shift, pain had worsened. By next morning, pain had increased causing miner to go to ER.

Miner accessed nose of drop deck trailer and fell ~4 feet to ground below and fractured left wrist.

Employee was cutting a slab of rubber with a dirty box cutter at Separator 2 area when EE cut self "on the last bit (end)" Employee was taken to emergency room where EE received wound cleaning and closure with staples to EE's right thigh.

Team member was assisting rolling pipes on the dredge. Tool became disengaged when team member went to pull down on handle, causing them to lose balance and fall backwards onto a metal box.

Team members replacing return roller, shaft began to slip, team member grabbed the shaft, causing pain to the right wrist.

Team member installing new exhaust gaskets, bolting up the flanges when debris fell onto and behind safety glasses, into right eye. Flushed eyes at the shop, debris remained behind eyelid.

MINER WAS SPOOLING IN A WINCH CABLE. EE WAS GUIDING THE CABLE WITH LEFT HAND AND CONTROLING THE FEED WITH THE CONTROLLER ON RIGHT HAND. AS THEY WERE ALMOST AT THE END, A U BOLT THAT WAS USED TO CREATE A LOOP AT THE END OF THE CABLE, CAUGHT THE CUFF OF THEIR GLOVE AND PULLED THEIR HAND INTO THE OPENING, THE U BOLT CAUGH THEIR MIDDLE FINGER AND LACERATED IT.

Employee was blowing themselves off with air hose after end of shift. Leading to foreign material getting into their right eye.

large rock got stuck in the jaw of the crusher. A TM attempted to use a chain to wedge between the rock and the jaw dies to break it up. As the rock and jaw pulled the chain down the TM tried to pull the chain up, resulting in smashing EE's hand between the chain and the railing causing laceration to left palm and a fracture to the 5th metacarpal

Miner was lifting a train door opening bar when the door bound causing EE to strain a back muscle.

Miner was manually opening a dump train car when material sloughed down onto the door causing it to rapidly open jerking the miner's arms causing a shoulder strain

An operator, needed assistance getting a loader out of the shop. They had a 10ft stand under the lift arm, supporting the bucket. When I went to pull the stand out from under the loader, I injured my back. I felt a sharp pain in my lower back immediately. I laid down for a few minutes, then left work to go to the chiropractor.

Individual attempted to lift a 5-6 ft. long chunk of I-beam (weighing approximately 150 lbs.) and toss it over the handrail of an elevated screen deck to the ground below. The I-beam shifted and pinched their right thumb between the beam and handrail. Individual complained of extreme pain and was transported to Urgent Care where an X-Ray discovered a broken bone in the right thumb.

The team member was assisting with the installation of a new Dredge bucket when their left index finger became caught in-between a becket and the work platform causing a laceration.

Miner was working on top of a water tank that was approx. 7' tall. Miner was removing an Air Compressor that inserts pressure into the water tank. As miner proceeded to remove it, the compressor suddenly propelled forward with enough force to strike miner on the face and chest area. Miner fell sideways to the ground hurting right leg.

WHILE EXITING SKID STEER, EMPLOYEE FELT POP IN STOMACH.

Was stepping off the track of the excavator onto step hearing a pop in their knee and fell.

Employee bent down and knee popped and shooting pain up employee's leg.

Member was closing train dump gate and strained a rib muscle

Employee scratched left eye with a paper bag.

EE illness is due to torching & burning metal while dismantling SEP01. EE did not report throat discomfort, coughing & congestion until 9/18/24. On 9/18/24 EE visited the ER & received treatment for acute bronchitis, chest x-rays were negative. Doctor prescribed medication & told employee to follow up with primary doctor. The EE was released back to work with no restrictions.

Team member was tying a boat up when their left hand was caught between the metal railings, resulting in fracture.

employee lifted the motor up to put it down where we could use the Crain to hoist it the rest of the way out. while lifting the motor employee felt something tear in the abdominal area. incident occurred 8/26/24 - it was reported 8/28/24 and found out EE was going to receive surgery 9/20/24 then surgery was moved up to 10/7/24

Employee was attempting to remove the swivel adapter from the auger head. A 36-inch pipe wrench was attached to the adapter and while attempting to rotate the auger shaft, the auger shaft unexpectedly rotated with the pipe wrench attached. The pipe wrench struck EE in the right chest and pushed EE against the back of the drill truck.

Team Member complained of chest pain before suddenly becoming unresponsive. Bystanders administered CPR and they were transported to the hospital.

Team member got dirt and a small cut on their left eyelid on 6/24/2024. By 6/26/2024, the eye was swollen and became painful. Team member sought medical care at a local eyecare center.

Team member was removing washer bearing from back of log washing machine. While pushing the bearing flange through the wall, their finger was pinched between the wall and the flange causing a laceration to pinky finger which required stitches.
