A roof fall occurred underground at xc 23 number 5 entry on the e. mains.

An unintentional roof fall occurred in the H7 Tailgate section, #2 entry, inby 36 wall. The fall was approximately 7' high x 16' wide x 30' long. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the fall. No injuries occurred because of this accident.

An unplanned roof fall above the anchorage point occurred in the intersection of crosscut 1, entry 2 of the 5 Left sealed off section.

A non-injury roof fall occurred in the #5 entry on the 2nd East Mains at spad #258. The fall was approximately 80 feet long, 20 feet wide and up to 50 inches thick.

An unplanned roof fall occurred on 5/4/26 at 8:22 am. The fall measured 16'by 40'long. The fall occurred in #6 entry 83 cross cut in 5 North. The area was posted on all sides and dangered off.

A roof fall was discovered at break 10, #2 entry of 78 HG panel. The size of the fall was approximately 12ft H x 16ft W x 20ft L.

An unplanned roof fall occurred on Unit #3 in the last open crosscut in between entries 4 and 5, crosscut #9 on the 1st NW Submain. The fall measured approximately 12 feet in length, 17 feet in width and 7 feet in height. The fall landed on the unoccupied roof bolter #6038.

A roof fall above anchorage was discovered on the weekly return examination in the number 2 entry near station spad 304. The fall measured approximately 90' long and 20' wide.

A non- injury unplanned roof fall occurred on the old worked out 1st North Submains return beginning at SS 3897 and extended for approx. 150 feet. The fall was approx. 20' wide and 2 to 5 feet thick. The fall was in the travelable return entry used by the weekly examiner.

The longwall operators were cutting high in the tailgate causing the shear to cut out roof support (bolts). This roof fall occurred in the tailgate (bleeder) no employees were trapped, injured, and air was not disrupted by the event.

There was an unplanned roof fall in the Panel district 11, 9th North, XC- 45, #5 entry.

A roof fall occurred in the longwall tailgate that went above anchor point. Ventilation was not interrupted, no miners were trapped, and no injuries occurred. The area where the roof fall occurred is a bleeder and is not a working area but the roof fall did extend passed the anchor point.

An unplanned roof fall occurred outby the section loading point above anchorage. The fall is located at #16 crosscut from the #7-#8 entries measuring approximately 90 feet long by 10 foot thick. Supplemental roof support was added in the adjacent entries, and the fall has been breakered off.

A roof fall occurred at this location on 03/27/2026. The fall location was on the 2nd East/4th North in entry 3 in the intersection at survey station 25+80. The fall measured 10ft wide by 15ft long by 2-3ft thick. No one was injured.

An unplanned roof fall occurred on 3/24/2026 at approximately 7:40 am on the slope. The fall occurred between crosscut No. 0 and 1 and was approximately 50' long x 20' wide x 5' thick.

A roof fall occurred at District #4 Headgate #1, #2 Entry, Crosscut #62 in the Intersection outby the Section Loading Point.

An unintentional roof fall occurred the front end of the previously mined 12L section. This fall was in the #1 crosscut between #6 and #7 entry. The roof in this area previously identified as an area of concern and was supported and dangered off. It has been noted that a stream valley runs though this area and may have contributed to the fault. approx 30'by 13.5' by 7' high.

As the airway examiner was examine the weekly intake airway a small 10'x10' roof fall that pulled 4 roof bolts was found just inby the fan house, the lifeline was rerouted to another entry, we submitted a plan to clean and rebolt the area that has been completed.

An unintentional roof fall occurred in the 2 South Mains 50w in the #3 entry. The fall was approximately 10' high x 16' wide x 60' long in the #3 entry and 10' high x 16' wide x 60' long between #2 to #4 x-cut. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the fall. No injuries occurred because of this accident.

An unintentional roof fall happened on the S3C-2nd Right entry 7 crosscut 7. It fell out in an intersection on top of a continuous miner. It is believed that the water above the stack rock deteriorated the rock strata causing it to fall. A site specific plan was submitted and approved to clean rock off continuous miner to retrieve it from the fall.

Unplanned roof fall discovered reportable in the old 11 belt entry, which is now common with 9 South track, at 82 1/2 block. Fall was approx. 10' high, 16' wide and 30' long. The fall will not be cleaned up or re-supported. No problems with equipment or compliance issues, training or PPE.

An unplanned roof fall occurred on 3/5/2026 at 11:50 am. The fall measured 16' long by 13' wide by 8' high. The fall occurred in #1 entry just in by 90 crosscut in the 6 South Mains. The area was dangered off and will be cleaned up.

Approximately 1:10am, a roof fall occurred on the #1 Section in #8 entry at 12cc 1cc inby spad #4379 trapping 2 Mobile Roof Supports (MRS's) in the fallen material. There were no personal injuries.

A non-injury roof fall was discovered in the scoop roadway 2 breaks inby punch out. Spad #382 across from fall in the belt entry. Fall measured approximately 25 feet long x 16 feet wide x 5 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.

An unplanned roof fall occurred on 2/16/26 at 8:10 pm. The fall measured 16'wide, 60' long and 4' high. The fall happened on the belt line at 3-36 xc and was dangered off and will be cleaned up.

On 02/15/2026 an unintentional roof fall occurred On the P3-1 (Unit #3) at crosscut #18, entry #3 in the last open crosscut. The fall is approximately 20 foot in length, 30 foot wide and 8 foot thick. It seemed the water bearing sandstone interval came down to approximately 8 foot, causing the immediate roof to fail.

The mine experienced a roof fall in the intersection of 40 crosscut on 1TG in the #2 entry. The dimensions were 19' x 20' x 14' (approximate). No injuries occurred from this incident.

An unintentional roof fall occurred on the 9HG LW section that measured 45'Lx10'Hx18'W located in the #3 entry between 13 -14 block. There were no injuries and the escape way was rerouted to the LW tail. A meeting was held with all miners as to be advised of the new escape way. Reference # 04950714

on 1/29/2026 an unintentional roof fall occurred on the #35 cross cut entry #5(belt line)of the 2C. The fall is approximately 20 foot in length, 28 foot wide, and 15 foot thick. It is believed that water deteriorated the upper strata making it unstable, causing it to fall.

There was an unplanned roof fall at crosscut 15 between entry 6 & 7 in the panel district 11 4th North.

An unintentional roof fall occurred at 6 South 101 x-cut between 7 & 8 entry. The fall measured 14ft wide and 16ft long. Initial height measurements did not indicate that the fall was above the primary support. Additional material fell causing this to become a reportable incident. The fall was caused by adverse roof condition in the form of deteriorated laminated shale.

There was an unplanned roof fall in the #9 2nd North Main XC-33 entry #3.

A roof fall occurred inby the loading at #58 crosscut, 2-Right on the District #4 Tailgate.

An unplanned roof fall occurred on 12/26/25 at 11:59 am at 34 3/4 crosscut on 5 north #1 entry. The fall length was undetermined at the time of the fall. The fall was dangered off and will be cleaned up and bolted.

A roof fall occurred at this location on 12/18/2025. The fall location was in the 1st panel south off the 3rd main west (Unit 4), entry 9, in the intersection at survey station 42+00 (71xc). The fall measured 22ft wide by 24ft long by 6-8ft high. No one was injured and ventilation was not affected.

A roof fall occurred at this location on 12/12/2025. The fall location was in the 1st panel north off the 3rd main west (Unit 3), entry 4, in the intersection at survey station 18+00 (31xc). The fall measured 18ft wide by 20ft long by 5-7ft high. No one was injured and ventilation was not affected.

There was a roof fall in 18H 3 entry xc 5 intersection. It fell on the belt line and monorail in the entry. The fall was roughly 10' high, 30" long, and 20' wide. No injuries, no trapped miners, and no disruption to ventilation on the longwall face.

A non-injury roof fall was discovered on the #2 Section 1 brk inby spad no. 575. Fall measured approximately 25 feet long x 15 feet wide x 8 feet high. Fall has been timbered off and danger signs posted on all approaches. This mine is in non-producing status.

Roof fall at 17hg return 3 entry xc 5. Roughly 10' high 20' wide and 30' long.

Hogg Contractor bent over to pick up hand tools. After EE stood up, a piece of draw rock dislodged from the roof falling striking EE on EE's right foot. This resulted in a fracture to the #3 and #4 metatarsals.

An unintentional roof fall occurred on the #21 cross cut entry #4 (travel way) of the 2C. The fall is approximately 30 feet in length, 27 feet wide and 10 foot high.

A roof fall occurred at this location on 12/4/2025. The fall location was on the 2E/4N between entries 3 and 4 at the survey station 10+80. The fall measured 17ft wide by 30ft long by 3-4ft thick. No one was injured.

There was an unplanned roof fall at #9 1st East Main between 10+40 and 11+50 in the #4 entry.

An unplanned roof fall occurred on 11/21/25 at 3:10PM at 56-57 xc #1 entry. The fall length was undetermined at the time of the fall. The fall was dangered off and will be cleaned up and bolted.

There was an unplanned roof fall in the #9 1st East Main at crosscut 17 in the #5 entry.

A non-injury roof fall occurred in the primary escapeway on 4 belt, entry 1, just inby crosscut 10. The fall was approximately 25'x15'x5-6'.

There was a roof fall in the tailgate of the longwall measuring 15ft to 20ft long and 7ft to 8ft high. The fall did not restrict ventilation, cause injury, or impede escapeways.

An unplanned roof fall occurred on 2 Gas Mainline 3 section at 38 crosscut, #3 intersection, in the last open break.

A roof fall occurred at 63 crosscut on 7B track. The fall was 4'high, 14' wide and 35' long. The area has been cleaned and re-supported.
