As the airway examiner was examine the weekly intake airway a small 10'x10' roof fall that pulled 4 roof bolts was found just inby the fan house, the lifeline was rerouted to another entry, we submitted a plan to clean and rebolt the area that has been completed.

An unintentional roof fall happened on the S3C-2nd Right entry 7 crosscut 7. It fell out in an intersection on top of a continuous miner. It is believed that the water above the stack rock deteriorated the rock strata causing it to fall. A site specific plan was submitted and approved to clean rock off continuous miner to retrieve it from the fall.

On 02/15/2026 an unintentional roof fall occurred On the P3-1 (Unit #3) at crosscut #18, entry #3 in the last open crosscut. The fall is approximately 20 foot in length, 30 foot wide and 8 foot thick. It seemed the water bearing sandstone interval came down to approximately 8 foot, causing the immediate roof to fail.

on 1/29/2026 an unintentional roof fall occurred on the #35 cross cut entry #5(belt line)of the 2C. The fall is approximately 20 foot in length, 28 foot wide, and 15 foot thick. It is believed that water deteriorated the upper strata making it unstable, causing it to fall.

Hogg Contractor bent over to pick up hand tools. After EE stood up, a piece of draw rock dislodged from the roof falling striking EE on EE's right foot. This resulted in a fracture to the #3 and #4 metatarsals.
