EMPLOYEE WAS HAULING COAL TO A PREPARATION PLANT WHEN FOR UNKNOWN REASONS LOST CONTROL OF THE TRUCK AND WENT OVER AN EMBANKMENT. EMPLOYEE WAS THROWN FROM TRUCK AND FATALLY INJURED. IT WAS DETE RMINED DURING AN INVESTIGATION THAT THIS WAS NOTDUE TO ROAD CONDITIONS OR MALFUNCTIONS TO THE TRUCK.
