WORKING AT VENTILATION WELL 192-B, EE WAS ASSISTING WITH THE RIG UP OF WIRELINE SERVICE EQUIPMENT TO PERFORM PLUGGING OPERATIONS ON THE WELL. THE POWER LINES OVER HEAD WERE REPEATEDLY SAID TO HAVE BEEN DEENERGIZED BY ISLAND CREEK PERSONEL ON THE MORNING OF THE ACCIDENT WHEN RAISING THE PORTABLE MAST ABOVE THE WELL HEAD, CONTACT WAS MADE WITH ONE POWER LINE THAT WAS STILL ENERGIZED.
