OUR DRIVER WAS ON TOP OF OUT SEMI TRAILER OPENING THE CENTER HATCH IN ORDER TO BE ABLE TO RECEIVE BULK CEMENT. IT APPEARS THAT THE DRIVER HAD ALSO TAKEN A TUBE OF GREASE TO APPLY TO THE LATCHE S FOR THE LID OF THE SEMI TRAILER. WHILE ON TOP OF THE SEMI TRAILER OUR DRIVER WITHER SLIPPED, LOST BALANCE OR SOMETHING, CAUSING DRIVER TO FALLTO THE GROUND.
