CONTRACTOR TRUCK DRIVER DELIVERED A LOAD OF BLACK "POLY" PIPE TO THE MINE SITE. UPON ARRIVING AT THE UNLOADING YARD HE DISCUSSED THE UNLOADING PROCEDURE WITH THE CONTRACTOR FORKLIFT OPERATOR. THE FORKLIFT OPERATOR STARTED POSITIONING THE FORKLIFT TO UNLOAD AND THE TRUCK DRIVER WALKED AROUND TO THE BACKSIDE OF THE TRUCK. AS THE FORKLIFT STARTED IN, 2 BUNDLES FELL, 1 ON TRUCK DRIVER.
