Employee was nailing up a fly pad with back to the rib when a piece measuring 66"x40"x22" fell striking employee and coming to rest on the left foot.
Employee was tramming scoop in muddy conditions when scoop slid against rib, causing the rib to roll out onto the deck resulting in multiple injuries. (left wrist-fracture / 3 sutures to chin / 2 chipped teeth).
Employee was bolting in #7 entry about 3 rows inby corner. While he was swinging pod out to bolt the corner, rib rolled out striking employee's left ankle. (2 bones fractured) Rib was 78" long x 39" wide x 32" thick. Rib was behind curtain. Employee was treated at Raleigh General Hospital.
A piece of rib (12"-6"-4") fell out and bounced off the ventilation tube and the continuous miner and then struck the injured in his left knee. The injured was bolting on a full face miner. The injured continued to work till going off for surgery on 1/27/2016.
Employee was operating the continuous miner when a rib rolled off pushing employee against shuttle car that was being loaded resulting in a fracture to his lower right leg.
Employee was leaning against a scoop when a piece of draw rock fell and struck his left pinky. Surgery was performed to amputate part of the first joint of the left pinky.
Employee was setting up to take 1st lift out of block. Injured bent over to throw miner slack (cable) next to rib, when without warning, Rib Rolled off and struck him on the lower left leg and ankle pinning him between the miner and the rock. went to hospital and received X-rays RX.
Injured was backing scoop around the corner to reposition scoop, when the corner rolled & a piece of rock fell in the deck of the scoop striking his right arm at the elbow resulting in a bruise.
EE was standing in the #8 entry looking outby when a piece of the coal rib measuring 86" X 25" X 6" rolled off striking him in the back
Employee was pulling cables by a parked shuttle car when a coal rib fell hitting him in his chest causing him to fall against the rub rail on the shuttle car resulting in a deep bruise on his left thigh bone. Employee was transported to Appalachian Regional Hospital where he was admitted for observation.
Coal rib sloughed off striking employee in the left leg.
Employee bent down to pick up bit buckets by rib, when he raised to get up a piece of rock fell from corner and hit him in the hip and knee area. He was treated by section EMT-M and taken to surface where he was transported to hospital and diagnosed with a contusion to the knee area.
The injured was operating the continuous miner when a piece of rib measuring 3' by 3' by 3" struck him in the right shoulder/scapula area. The injured has continued to work at his normal job and continued to have pain in his shoulder. An MRI revealed some tearing in his shoulder. The injured began receiving physical therapy on 11/24/2014. This is a Medical Treatment Accident.
EE was knelt down behind the miner picking up pull ropes that had fell off. The rib rolled out and struck him on the back of the head and his back. The rock measured approximately 80" long X 17" wide X 11" thick. He was transported to Raleigh General Hospital where x-rays were negative. He was released to return to work. Due to continuing pain unable to RTW on 10/8/14 & 10/9/14
Repairman was in the process of cleaning the section scoop when material from the rib slid down coming in contact with his left foot and ankle.
Employee was rolling a rock from in front of the high voltage sled to rib. He was approximately 2' from the rib when it rolled out and onto his right leg. He suffered a fracture to the medial malleolus, which is the small upper bone of the inside ankle. he was place on restricted duty for 6-8 weeks.
Employee was working on the 001 MMU in the # 8 Heading, he finished installing his inside bolt and the right rib rolled out, a portion of the loose rib struck his right calf muscle. He refused an ambulance and was taken to the Logan ER and diagnosed with a hairline fracture to his fibula.
Employee was lapping belt on 43 break on #5 belt and a loose rib fell out onto his right leg.
Employee was operating continuous miner in #5 entry when right rib rolled out striking him in the back and pushing him over into shuttle car rolling his right ankle and injuring his lower back.
Employee was helping electrician remove rub rail on miner when a piece of rib fell and struck the right side of his middle to lower back. Employee continued to work until 11-18-2013 where he was taken off work due to his illness.
Employee was operating miner when the rib rolled off of the corner striking him on the right foot. Fracture to the right foot.
Operating CM with remote and rib fell pushing him against machine lacerating left side of upper forehead. The laceration required 10 sutures to close. No time was missed.
Removing ventilation tubing and was struck by a falling rib 2 feet x 3 feet x 1-1/2 feet thick that struck him on the left side of his body on the shoulder, arm, leg and knee. He continued to work until 12/16/13 at which time he was taken off work for surgery to his left knee for a meniscus tear.
The employee had just finished scaling down loose rib when a piece of rock fell from the rib striking his left arm/wrist causing a laceration and bruising.
The employee was reaching across the top of the miner to retrieve a box of glue when a rock came loose from the face striking the straps that were being stored on top of the miner. One of the straps struck the employee under his right arm. There were no cuts/contusions to the employee and the physician's diagnoses was a strained right shoulder.

Employee was arranging the line curtain near the intersection at the No. 7 face when a rock rib rolled off the corner and struck him on back and side.
Hanging cm cable and water hose in a cable handler and a piece of rib fell striking him on the head, neck, and shoulders and knocked him to the mine floor. He began losing time his next scheduled shift on 10/08/12.
The employee was operating a shuttle car when a coal outburst occurred. The flying debris from the outburst struck the employee in the face and head which resulted in a fractured skull with fractured facial bones.
Employee was sitting on a plaster bucket on the in by end of the #4 entry block on the corner writing notes when a section of the coal rib dislodged without warning, sliding down the face of the rib and striking him in the back and knocking him to the mine floor. The rock measured 63" long x 25" wide and 17" thick.
Employee was operating Miner in #3 entry Face, He was making his clean up run, when a rib rolled out striking him on his right side and Right foot.
Employee was sent to clean #8 Entry, He was found lying on the ground along the left rib with 2 pieces of Rock Appx 14"x14"x3" laying on employees left thigh area. Look as if Rib rolled out on him
Employee was recovering curtain when rib rolled and struck employee in back.
Employee was checking the MRS cable on the offside of the miner when a rib rolled striking his right lower leg.
The EE was moving the miner over to his next cut and the corner of the rib rolled off and hit him in the back of the lower leg. EE was not able to return to his regular job.
The miner operator attempted to get the cable out of the roadway. A piece of rock rolled out of the rib striking him on the foot. He visited the ER and was found to be bruised. He was given a full release to return to work and did so. On 12/16/11 he visited specialist. Dr determined he had a hairline fracture in one of the metatarsal bones in his foot. He began missing work.
Employee was setting the bolter up for the first row in 7 Right. He was at the controls on the operator's side when the rib turned loose and the rock fell on its edge catching his right foot. The rock was 69" long, 24" wide at the widest point and 6" at the narrow end. It was 11.5" thick at the thickest point and 6" at the thinest. This resulted in multiple fractures in his foot.
Employee was driving a spad nail into the rib to hang a cable when the top of the rib rolled out striking him on the right ankle resulting in a fracture. Rock was approximately 0-6" thick by 39" long by 8" wide.
Employee, scoop operator, was rock dusting in #6 entry, operator had his hand on the top of scoop door when scoop quit tramming. Employee recycled the tram pedal and scoop moved forward striking rib, a rock fell striking employee on his little finger of left hand smashing it against scoop door. The little finger was damaged to the extent that it has to be amputated
Employee was installing roof bolt when a coal rib (2' long, 18" wide, and 2" thick) rolled off and hit his left ankle. Coal fell in loose pieces and accident occurred in #6 entry 150' inby SS# 2055
The miner operator was repositioning the miner in the #8 entry on 002 section. As he was setting the miner over, a piece of rib fell out and struck him on his right leg. Employee was attended to by to M.E.T. on the section and transported outside. Employee has a possible fracture to right leg and ankle.
Roof bolting operator was putting up the outside bolt in the first pattern row. The coal rib rolled out due to the canopy resting against the rib while operating. The rib roll struck the operator against his left side of his back causing lacerations. (Entry Width 17'11") (Rib Roll 8" thick - 15" wide - 20" long) (Hole being Drilled 26" from the rib)
EE was repairing "dog bone" on the face side of the pan line when multiple rocks fell between the shields striking the EE in the right leg. The EE received 6 sutures in the right leg. On 9/19/11 injured began losing time due to infection of the laceration. He had not missed any time before this.
Employees had scaled some loose ribs in an old overcast area and was in the process of spot bolting the area. As he was putting the drill steel against the roof a piece of rib fell out knocking employee into the roof bolter controls causing a laceration to his face.
Employee backed the miner out of place and bumped rib with the miner pushing corner of turning rock loose over top of the coal. The rock struck him on the right side resulting in a broken foot.
Employee was bolting in 6 right when a piece of rock approximately 24" x 3" x 2" fell from the rib striking employee on the lower back. Employee was treated by EMT's at the scene and transported by Whitesville Ambulance to CAMC General Division. Employee had xrays taken which was negative but was referred to a spine specialist for further treatment.
Employee was running miner in No. 1 heading to right hand lift when a piece of rib rolled off striking him on lower back and across both ankles. Accident occurred at MMU 001, #1 entry 20' in by spad #19641. Rock was 12' 2" long, 25" wide and 12" thick.
Wrapping ventilation tubing and turned to get sidecuts when he was struck by a piece of rib approximately 26 feet long x 6 feet high and up to 1 foot thick. He received a fractured left femur.
As employee was loading shuttle car the rib rolled and struck him on his right hip resulting in a fracture.
