EE was tracking Conveyor belt, and saw an Idler not rotating. EE used their hands to try to break the Idler free, The roller starting rolling which resulted in them being pulled into the Idler. their left arm was pulled between the Idler and belt.

Employee was operating a loader when they alleged they hit a pothole that jarred their back.

Employee claimed that EE hurt EE's shoulder while digging in the muck pile with the pit loader.

Employee was in a haul truck when a rock fell and jarred the truck. Employee complained of back pain and utilized the RICE treatment until the January 24th. Up until the 24th, the employee stated multiple times they were feeling better each day. Employee went to ER with back pain and was released with prescriptions.

Employee hauling wet plant waste from plant area to dump with CAT 775D haul truck with tailgate. Employee began traveling on primary roadway when the weight of the plant waste shifted towards the back of the truck bed resulting in the front end lifting off the ground. When front end came back down it bounced twice before coming to a halt.

Employee was operating a Loader when they drove over uneven ground, causing pain to their lower back. They were diagnosed with a back strain and issued restrictions.

EE was operating a loader while working on a ramp in the plant yard. The bucket was fully loaded, and fully raised when the loader tipped over on its side.

Employee was running skid steer when the machine came to a sudden stop. The employee's momentum did not stop and EE hit EE's head on the rear view mirror, causing a laceration requiring stitches.

Employee alleged that they injured their shoulder while running the loader on 6/2/2025 but did not report this injury until 7/2/2025 and became reportable due to restrictions on 7/3/25.

Employee was descending into the pit when they lost control of the water truck they were operating. Employee attempted to turn up an old ramp and overturned the truck. As a result, the employee sustained two fractured vertebrae.

Employee got EE's arm caught in a troughing roller on a conveyor.

Employee stepped out of the cab of the truck to clean the mirrors when EE noticed the truck was rolling. While re-entering the cab, EE's left hand was pinched between the cab handrail and the truck bed in front of EE.

As the employee was using a skid steer to move grits in the plant the machine hit a rut and bounced and the employee claims it jarred EE injuring EE's back.

Employee pulled away from office in service truck and was struck in passenger door by customer truck. On 12/12/2024, employee sought medical attention where management became aware EE was given physical therapy order.

Employee stated EE had to drive on rough roads and was sore as a result of it. Later diagnosed on 5/1/24 put on restricted duty and is still being evaluated.

Employee was accessing a secondary crusher platform to inspect a crusher when a rock fell from an overhead conveyor striking the employee in the back of the head.

On 2-16-24 employee (EE) in haul truck was being loaded. Bucket of the loader made contact with the bed of the haul truck. The shaking of the truck caused discomfort to EEs back. EE was provided first aid and returned to work with some soreness but was able to drive without discomfort. On 4-15-24 EE went to the clinic due to pain when sleeping and was issued restrictions & therapy.

Trespasser wrecked a four wheeler within the plant's stockpile area.

A Komatsu 605 haul truck backed into a Cat 775G haul truck. The right rear corner of the bed of the 605 contacted the left side handrail and front left cab support of the 775G. EE in the 775G was transported by ambulance to ER claiming pains in their back.

Employee was riding in the buddy seat of an articulating truck when the truck hit a rock, jarring the employee's back.

Employee alleges that while operating the water truck on 8/25/23, employee hit a bump causing a back pain. Employee did not report the incident until 09/05/23 and sought medical treatment on 09/06/23. While this incident is still under investigation, this 7000-1 is being reported within the 10 business days of receiving medical treatment.

Employee alleges the seat in 29062 EH 1600 was "bottoming out" when hitting road bumps. Employee alleges neck and shoulders strain.

A non-fatal accident occurred on October 10, 2022, at approximately 08:55. A customer truck was leaving the property when customer drifted off the paved road causing the dump truck to roll onto its passenger side. It took EMS greater than 30 minutes to remove the driver from the truck.

Employee was cleaning up with S925tx. The employee stated material fell from the conveyor frame striking the employee.

Employee was attempting to free a rock at the back plate of a conveyor and the belt pulled the employee's hand into the back plate of the conveyor.

Employee struck EE's left lower back on a conveyor support frame. Employee received bruising on EE's back.

EE was operating a haul truck and lowered the haul truck bed. EE claims when lowered it jarred EE's back.

Employee lost control of the articulating haul truck, veered right onto the right side berm and overturned the truck.

Alleges neck was hurt when the loader operator dropped the boom on side of haul truck. Did not seek medical treatment until 5/5/2022.

Pit loader operator in a 992K loader, drove parallel to the muck pile. A large boulder slid down the pile and struck the right front loader tire. The tire blew out and during the movement the employee's right hand was injured by an unknown object in the cab. Employee suffered from a laceration to the right thumb and a strain to the right thumb / palm area.

Employee was digging in the muck pile when employee alleged that the seat belt hurt abdomen.

Employee was running a grader when a bin truck ran into EE. EE felt pain in EE's shoulder but did not immediately seek medical attention. On December 15th EE was diagnosed with a bicep and rotator cuff tear. EE will have surgery at the end of December and will become a lost time accident.

Employee was traveling in a man lift and hit a pothole in the road causing EE to get jarred in the basket and claims EE hyperextended their knee.

The employee was pinned between right-front tire and frame on the 775D haul truck.

Haul Trucks were hauling ½"- rock from the Stone Plant to the Asphalt Plant. Approx. 10:40am, the operator of the empty haul truck was traveling S. toward Stone Plant. The empty truck appeared to travel outside its lane and collided with the cab of oncoming loaded haul truck traveling N. Both EEs were transported and admitted to hospital due to multiple injuries.

Haul Trucks were hauling ½"- rock from the Stone Plant to the Asphalt Plant. Approx. 10:40am, the operator of the empty haul truck was traveling S. toward Stone Plant. Employee was scaling in a scaler when the cab lift cylinder bolt sheered off causing the cab to drop suddenly jarring employee in the cab. It fell approximately 13 inches where it hit a cab bumper block. The employee claimed to have a sore back and neck. The supervisor took the employee to the clinic and was diagnosed with a strained lower back and neck with restrictions.

Employee was in a Komatsu HD605 Haul Truck. Employee was headed toward the 57 pile when they over shot the turn onto the ramp striking the berm at an approximate 45 degree angle. When the right side front tires contacted the loose material along the berm, the truck continued sliding through resulting in the truck laying over to the right side.

This claim has been denied by W/C for inability to provide specific event or reliable date. Employee alleges back soreness / aggravation over time possibly from seat in 988H Loader #54. Employee claims back has not improved over time. Sought medical treatment on 6/10/19, prescribed physical therapy on 6/14/19.

Employee struck a large rock with F550 lube truck while driving in the pit resulting in a laceration to EE's lips.

EE was at pipe rack as fork lift was picking up a piece of steel pipe to put onto the pipe rack when the pipe rolled on the ground and onto their foot crushing the foot

Employee exited the company vehicle to talk to supervisor, who had pulled up beside EE in their vehicle. Employee's right leg was pinched between EE's door and the other truck door. Injury became reportable on March 12, 2019 when employee was prescribed an ankle/foot orthosis.

Employee reported while driving a haul truck that it hit a pothole and bottomed out causing pain in employee's back. Employee reported the incident to employee's supervisor on 7-20-2016 but did not request to seek medical treatment until 8/4/2016. Employee was diagnosed with a back strain. Employee was able to continue working until 1/27/2017.

Employee's body shifted in the cab of haul truck when EE ran over a pot hole. A short time later EE experienced numbness in legs and was taken for medical treatment. EE is currently losing time.

Employee was traveling to work on personal motorcycle. After employee went through the plant entrance, ran over a slight hole in the pavement and lost control of motorcycle, injuring knee.

Employee washed out bed of articulating truck in plant and did not lower bed as EE drove off the raised bed contacted the conveyor protection bridge causing the cab to lay over on its side.

Employee was operating a haul truck when it was struck by a loader bucket.

On 11/10 Haul Truck Driver complained of back pain at the end of his shift. The employee reported his seat bottoming out while driving. The mechanic inspected the seat and determined the seat was fine. On 11/23 he requested medical treatment and returned to full duty. On 12/18 he saw his personal doctor and taken off work for 2 weeks.

On 9/7/2015 a small passenger plane carrying three people crashed in a graded area on our property. Unfortunately, no one survived the impact. Authorities were notified and the scene secured. Vulcan personnel called the MSHA 800 number immediately after confirming there had been fatalities related to this incident. A courtesy call was also made to the field office supervisor.
