Mostrando 50 de 264Employee was injured when they fell walking to the troop carrier. Injury became reportable on 11-18-25 when an orthopedic specialist diagnosed a left shoulder partial rotator cuff tear and left shoulder proximal humerus fracture. Physical therapy was also ordered.
Employee rolled left ankle resulting in a fracture
(Illness) Employee was standing and passed out, fell to the ground, and received a cut on EE's head from the ground. (total time passed out was less than a minute). EE was taken to the hospital, received 3 stitches to close the cut along with IV's. EE was then released, told to stay home one day, permitted by the doctor to drive self home. EE has been working since then.
Trespasser entered mine property and while attempting to climb down from bench 5 to bench 6, slipped and fell approximately 40 feet breaking both legs.

Employee was climbing a ladder to complete a task on the crusher. Employee went to step on to the third rung of the ladder and slipped. Employee landed on the handrail to EE's right. When employee fell EE's finger/arm caught a piece of metal plate resulting in a scratch on EE's arm and a cut on EE's finger resulting in the nail falling off.
While dismounting a loader, the employee slipped on the bottom rung of the loader and ruptured their left bicep tendon as EE's weight shifted.
12/10/24 EE slipped and fell on ice. EE continued first aid treatment until 1/27/2025 EE was prescribed psychical therapy.

Employee slipped on the concrete floor and fell on EE's left side. Physical therapy and restrictions were prescribed for the employee. Injury became reportable 4/26/2024 when the MRI validated the injury was work-related.
Employee was walking to check on pit pump and tripped resulting in a shoulder strain. Accident became reportable on 11/15/2024 when employee had surgery.

The injured employee (IE) was participating in first aid/CPR/AED classroom training when IE experienced a personal medical event and subsequently fell to the concrete floor striking face requiring medical treatment beyond first aid. IE was transported to a local ER where IE was treated for a nasal fracture and sutures to the nose.
The injured employee (IE) was operating a CAT D8T dozer to push material to the loader operator. As the IE was exiting the D8T dozer, the IE's heel was caught on the track causing IE to fall to the ground level. The IE was evaluated at the local ER and determined to have a fracture to IE's L3 disc. No further treatment was necessary.
While performing crusher maintenance, employee stood on mid-rail of handrail and fell to lower level.

Field machinist was carrying a boring bar out of the back of the service truck and going down stairs EE tripped and fell forward. The weight of the boring bar smashed EE's hand at impact to ground. Bleeding and swelling of EE's right hand was treated with triage, so first date became reportable was 4-23-21 when we were notified of the fracture.
Mine operator was notified approximately 4 hours later of a customer truck driver that slipped and fell on the ground.

Employee was on the bottom step of a four step ladder. Upon stepping down to the ground, EE lost balance and fell.
