EE was helping remove a stuck skid steer bucket. When the bucket dislodged it fell onto the EEs leg causing it to break.

Employees was working on service truck crane and EE step down off the truck, following the three points of contact. When the employee placed EE's left foot on the step and shifted EE's weight over to that foot to finishing stepping down from the truck, the employee heard a pop and felt pain in their calf.

On 7/09/2026, approximately at 1:15pm, an employee was working on the Chevy 2500 pickup truck to remove the bed. Employee said that EE hurt EE's knee while working under the truck.

Employee was using folding utility knife to remove broken plastic fitting from tube. Knife folded and cut employee's right index finger. Employee received 3 stitches

Employee was walking on ground floor of finish mill building. Cement dust fell from a level above them. Cement dust got into left eye causing irritation.

EE was attempting to free material with an air wand and felt a pop in right shoulder area. Reported late because no pain was felt for a few days.

On 6/24/2026, approximately at 10:57pm, an employee was working at our Pressure Deferential (PD) Rail Car unloading station when they tripped between track 4. The employee was wearing a hard hat as required, it came off during the fall and subsequently the employee struck the right side of their head on something. The employee sustained abrasions to the forehead area.

Employee took an audiogram test on 5/26/2026 and had a reportable result for EE's left ear. Employee was scheduled to complete an audiogram retest on 6/29/2026. Employee declined to be retested on that date.

The employee was taking a busted bag off the conveyor belt and as EE tossed this bag in a wheel barrel the material in the wheel barrel blew up into EE's face getting under EE's safety glasses and causing and eye injury.

EE was pushing a welder into the elevator when EE strained EE's neck and shoulder.

EE was welding hopper chute standing on belt rollers when EE lost EE's footing and twisted EE's left knee.

Employee had machine properly locked out. EE was removing a cover to clear a jam in the machine. EE dropped the cover on EE's right middle finger causing fracture.

Employee alleges EE stepped off a step and jarred EE's back. No unique conditions associated.

Employee removed radiator cp on mobile equipment causing minor burn to right hand

Employee was using an electric stair cart to move a 55-gallon drum of oil. The cart tipped forward causing employee to strain their lower back. Employee was released back to work with temporary restrictions. No bending, lifting, carrying, pushing, pulling, kneeling, squatting, or twisting.

EE was attempting to use pry bar to lift up shaft on roll press roll. As EE attempted the pry bar slipped and EE fell on right side, EE placed left arm to catch self and felt a sharp pain in left bicep.

Shotcrete pipe moved and pinched hand of employee.

The EE was doing clean up when a clay ball rolled off the feeder and rolled down the berm and hit their left wrist.

originally Diagnosed with a non reportable strain to EE's left knee, sustained while connecting a hose to a tanker truck. On follow up with Orthopedic Dr. 02/12/26 EE was scheduled for MRI and PT. Case elevated when EE started medical treatment (physical therapy) 03/09/2026.

EE was operating loader when the left front tire slipped off a slab approx. 8-12" and jolted operator in their seat causing a back strain.

Miner was conducting a mill return to service checklist, when EE stepped on a piece of clinker and felt pain in EE's right foot. Miner was prescribed a walking orthopedic device and restricted duty.

EE found that the doors for the clinker elevator would not open at top landing. EE used the descending device on the elevator to bring themselves down to ground level.

Miner was attempting to clear an airside that was plugged, when material blew back into their face causing an abrasion to the cornea. No foreign body in eye.

Miner was completing work on a table in the lab, when EE dropped an item on the floor. As EE bent over to retrieve the item, EE struck the right side of EE's forehead on a corner of the work table.

The Supervisor had placed their left hand in the line of fire for the 590 gate pneumatic cylinder. When the cylinder change positions their left hand was pinned while hold a flashlight.

EE was climbing off the dozer and went to sit their cooler down on the track and slipped falling and hitting their head.

EE was laying concrete and got it on their leg. EE was took to DR. and was treated.

At 11:20pm, a non-conforming item went through high speed shredders at Green America facility which resulted in a fire. Contacted MSHA at approximately 1130pm to notify them of the incident. Fire department arrived on site at 12am to assist with putting out the fire.

Employee was walking the plant all day collecting vibration data with a iPad and collapsed unexpectedly. 1st responders performed CPR, used an AED, & EE was transported to hospital. As of 9/30, remains in the hospital with double pneumonia, diabetes, & 3 blocked arteries to requires surgery. MSHA investigate the same day. No environmental or work related cause was found.

Employee had finger impacted in a pinch point of a dump hoper.

Employee grabbed a master link that was set on flange above EE's head and pulled cement dust into EE's left eye causing a chemical burn to the cornea.

Operator saw flames on conveyor heading towards GAR bunker and radioed and contacted control room to shut down system, while operator pulled estop cord. Fire department was contacted immediately. Arrived on sight at approx.02:15 am. Plant fire suppression system went off, fire department stayed on sight till approx. 04:15 am to make sure fire was extinguished.

Employee was initially injured on 7/8 at 0830am as EE tweaked EE's shoulder. This was initially treated as a first aid for a strained left shoulder with no work restrictions. Based on radiation of symptoms to EE's right upper extremity and lack of pain improvement, an MRI was ordered which resulted in the reclassification from first aid to recordable was made on 8/6.

Employee was attempting to lift elevator door on man elevator and door stuck which caused EE to strain EE's back. Was still working at full duty, became a recordable on August 20, 2025. Prescribed medications and also limitations. Able to work with limitations.

Worker was placing bag on fill spout and put their finger under the bag clamp. The clamp crushed the tip of their right ring finger causing an open fracture of the tuft of distal phalanx of the finger.

employee was going checking air hoses on a truck. when EE went to step up onto the platform, the step broke and the employee fell into the trailer injuring EE's right arm.

Driller was in the process of drilling boreholes, dust hose collection was not operating properly. Upon inspection of the dust collection hose, driller saw an obstruction. EE removed the hose to clear debris from the hose and upon re-attaching the dust collector hose, EE's right hand came in contact with the metal fastener on the hose, cutting EE's right hand.

EE was going up to the top of the upper shelf lime silo to unclog and get it to flow when EE opened the smaller round door to probe the lime or rodding if you will, it broke loose and came out of there like water getting all over EE and into EE's right eye.

EE was straightening out the hose on the grease gun to the drum. When EE was walking toward the steel portable silo legs, EE tripped over the steel legs on the ground and hurt left hip. The employee fell to the ground and said that EE couldn't get up and we called for an ambulance to assist.

Heat Exhaustion, while stacking sand bags

On 5/6/2024 at approximately 10:30 AM EE was in the shop when EE informed the plant foreman that EE wasn't feeling well. EE stated that EE felt a little dizzy and that EE's left arm was numb. EE was immediately transported to the hospital.

Employee was removing a broken pallet from the turntable on our production line. The employee slipped on the rollers on the turntable, and strained lower back.

Employee was exiting the crusher control room and as EE proceeded to come down the stairs EE's right foot slipped causing EE fall. When employee fell EE's left foot got twisted and ended up underneath EE causing the ankle to break in three different places.

As an employee entered a haul truck and sat down the employee felt discomfort in left calf. Clinic determined that the strained left knee.

Poor engineering. Employee's finger was smashed by swinging door.

first aid incident occurred on 10/30/23. No medical treatment prescribed. After months of continued pain, employee asked to be seen by doctor again. Doctor prescribed medical treatment (physical therapy,) elevating the first aid case to a reportable accident.

Employee was cleaning up cement that was on the floor under the cement line palletizer and was bent over using a come along to pull out the cement that was under the cement line palletizer when EE stood and turned and fell and twisted EE's knee on EE's left leg.

Employee reported to work with pain in shoulder, elbow and wrist
