AT APPROXIMATELY 7:45 PM ON FEBRUARY 14, 2000 DECEASED EMPLOYEE, PREPARATION PLANT OPERATOR, WALKED FROM THE PREPARATION PLANT INTO THE ACTIVE WORKING AREA OF A 275C MICHIGAN FRONT END LOADER. THE OPERATOR OF THE FRONT END LOADER WAS UNABLE TO SEE THE EMPLOYEE. HE WAS STRUCK BY THE BUCKET OF THE 275C FRONT END LOADER WHILE THE OPERATOR WAS FILLING THE BUCKET OF THE FRONT END LOADER
