Investigator narrative
The contract employee was helping to install a counterweight on Gradall XL5110 Scaler. As the weight was lifted into place, one of the connector links of the chain used for lifting broke, causing one end of the weight to strike the ground. This in turn broke the other end, which allowed the weight to strike the employee as it fell.
