FATALITY. NO GUARD ON OPENING OF AN ELEVATED WALKWAY. VICTIM FAILED TO FOLLOW COMPANY FALL PROTECTION PROGRAM. DID NOT WEAR SAFETY HARNESS & LANYARD & STEPPED THROUGH OPENING IN THE WALKWAY. FALL PROTECTION SIGNS WERE PLACED IN ALL AREAS WITH HAZARDS
