WHILE ATTEMPTING TO INSTALL BOLTS IN THE ROOF, EMPLOYER HIT THE UP LEVER ON THE BOLT MACHINE. HIS HEAD WAS OVER TOP OF THE POT & IT CRUSHED HIS HEAD AGAINST THE CANOPY.
