Investigator narrative
EMPLOYEES WERE CHANGING BOLTS ON THE ""DUTCHMAN"" ON AN ELECTRIC SHOVEL. THE DOOR OF THE DIPPER WAS BLOCKED OPEN WITH A METAL BAR THAT WAS WELDED IN PLACE. FOR REASONS UNDER INVESTIGATION, THE BLOCKING BECAME DISPLACED AND THE DOOR CLOSED, PINNING TWO EMPLOYEES BETWEEN THE DOOR AND THE DIPPER.
