OUR EMPLOYEE WORKING AT MOUNTAIN AGGREGATES QUARRY MINE SITE WAS FATALLY INJURED WHILE OPERATING A DOZER ON AN ACCESS RD IN WHICH THE EQUIPMENT LEFT THE ACCESS RD AND ROLLED DOWN AN EMBANKMENT.
