Investigator narrative
Employee was changing right lift cylinder on the Gehl skid steer loader. Employee is believed to have been entering the cab when the fork dropped to the ground. The employee had his head caught between the frame of the skid steer and the arm to raise the forks.

Trench was dug to repair drain pipe. No sloping or shoring protection system was in place. Trench was approximately 10 foot deep and 8 foot wide. Side wall collapsed and crushed worker.
