A FATAL ACCIDENT OCCURRED AT THE 5520-237 LONGHOLE STOPE WHEN A WATER TRUCK OPERATOR BACKED OVER THE BERM AND INTO THE STOP WHICH AT THE TIME WAS BEING BACKFILLED.
