Investigator narrative

While in the process of installing permanent roof support in the No. 1 Entry, the roof started to fall and the employee ran inby the ATRS System. The roof fell in this area causing crushing injuries to the employee.

The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.
