MAN PLACED HIS BODY (HEAD & CHEST) THROUGH TOP AND MIDDLE BARS OF RAILING AROUND A HOIST WELL COXX ON CONE FLOOR OF PREP PLANT TRYING TO FREE A LADDER WHICH WAS STUCK IN HOIST WELL, WHEN LADDE R FREED, MAN FELL TO FIRST FLOOR APPROX 49 FEET, HE WAS TAKEN TO HOSPITAL WHERE HE DIED AT 12D20 ON 6-9-84
