THE INJURED MAN WAS REPAIRING BUCKETS ON THE BUCKET ELEVATOR WHEN THE MILL OPERATOR STARTED THE BUCKET ELEVATOR. THE MILL OPERATOR WAS NOT AWARE OF THE WORK BEING DONE ON THIS ELEVATOR DUE TO A LACK OF COMMUNICATION AND THE EQUIPMENT WAS NOT LOCKED OUT OR TAGGED AS REQUIRED.
