VICTIM WAS OPERATING A DIESEL POWERED TRACTOR WITH A FRONT END LOADER BUCKET MOUNTED ON THE TRACTOR. ROLL OVER PROTECTION STRUCTURES WERE NOT INSTALLED ON THE TRACTOR. THE VICTIM WAS LIFTING A SMALL WOODEN HEADFRAME INTO POSITION OVER THE COLLAR OF AN INCLINED SHAFT (TOTAL WEIGHT 1,000 POUNDS).
