5:00 SUNDAY AM,I.NAME & I.NAME WENT TO MINE IN ORDER TO GET 2 DOZERS RUNNING FOR THAT DAYS SHIFT.AFTER I.NAME LEFT I.NAME WENT TO ROAD GRADER FROR SOME UNKNOWN REASON.BLADE WAS PARKED AT UPPE END OF 8% HAUL ROAD AND PARKED SO IT WOULD RUN INTO BANK IF IT GOT AWAY.SOMEHOW I.NAME GOT BLADE HEADED STRAIGHT DOWN HAUL RD AROUND 90 DEGREE CURVE AND DOWN 4500\ OF RD WHERE HE TURNED BLADE

    EE and another employee were pulling the engine and removing nose cone on the radiator. EE was on the bottom under the 992g wheel loader. The other employee was on top of the machine. EE was taking the radiator nose off, accidentally took bolts out of counter weights and it fell on EE. Resulting in a fatality.
