WHILE DRILLING 5T THE FACE OF THEMINE THE JACK HAMMER AND STEEL USED FOR DRILLING SLIPPED FROM THE START OF NEW HOLE INTO A PREVIOUSLY DRILLED HOLE THAT WAS LEFT FROM LAST CUT THAT DID NOT PUL L TO THE END DE7ONATED A PEICE OF POWDER RESULTING IN AN EXPLOSION AND CAUSING THE DEATH OF EMPLOYEE.
